EVIDENCE-BASED PRACTICE PROPOSAL 1
Evidence-based Practice Proposal
(Resolution of an Issue Significant to Improving Patient Care)
EVIDENCE-BASED PRACTICE PROPOSAL 2
Abstract
Evidence-based practical proposal is an important aspect for any organization in order to expand
valuable information to the related stakeholders. Therefore, this paper has an aim and objective
to explore an issue within the healthcare organization and solution to mitigate the problem for
safe and secure health care of the patients. The need of keeping record of the patient is termed as
useful consideration, but how it should be managed effectively is a big question. However, there
is a solution to manage personal information of the patients for good health care services and
eputation of the institution.
This paper is based on developing a full-blown resolution of an issue or problem significant to
improving patient care in a healthcare organization. Alternatively, an effective resolution of an
issue is basically related to increasing knowledge and understanding that how a healthcare
organization gets success in providing safe and secure patient care services. Therefore, this paper
would explore the benefits of the EHR system and implementation in the present scenarios for
the healthcare institution.
EVIDENCE-BASED PRACTICE PROPOSAL 3
Introduction
Healthcare organization is an important place where people go to achieve health care
enefits and well-being in case of needs. Further, healthcare organizations also play the most
important role in making a good relationship between healthcare providers and patients.
However, in most of the situations, issues in the healthcare organization can be seen due to weak
management system (Barzi, Mikhail & Shabot, 2012). There could be various types of issues in
the health care institutions/organizations, such as mismanagement of record keeping of the
patient, lack of appropriate hygiene, or cleanliness, careless patient care, etc. These issues are
enough to destroy the value and reputation of the healthcare organizations. The trust of the
patients, such as individuals, groups, families, or communities can be lost gradually and there
would be financial harm of the healthcare organizations.
At this stage, it is essential to remove the problems of the healthcare institution as soon as
possible in order to maintain good relationship and trust with the patients in terms of improving
patient care. In this consideration this paper is selected for the resolution of an issue or problem
that is significant to improve patient care in an organized manner (Bosek & Savage, 2007). This
paper is significant for the scholars and other readers to increase their knowledge and
understanding about the evidence-based issue and solution in the healthcare organizations.
Problem
The problem that is selected in this paper is associated with the patient record keeping
that is known as a crucial aspect of the healthcare delivery system. Further, patient record
keeping is usually noted as the most important part of the healthcare organization in order to
improve health and well-being of the patient (Collins, Stepanczuk, Williams & Rich, 2016). In
this, it can be often observed that wrong policy and approach of the healthcare management put

the privacy of the patients at stake in terms of record keeping of the patient. In this way, it has
put the reputation of healthcare institutions in jeopardy in the market.
At the same time, the wrong approach of the management system towards record keeping
and an inclination towards sticking to conventional paper record keeping system has put many
healthcare institutions in litigations in the past. This system has threatened the market reputation
of the most of the healthcare institutions. Hence, it has now become imperative for healthcare
organizations striving to achieve excellence in patient care giving to resort to the system of
medical record keeping of patients electronically (Collins, Stepanczuk, Williams & Rich, 2016).
In this consideration, healthcare organizations can install and implement the electronic health
ecord (EHR) system as a solution to the problem of
eaching of patient privacy in the course of
providing medical care.
The problem of
eaching of personal information or privacy of the patient due to
improper maintenance of private records in the course of care giving needs to be addressed
properly by the organization. There are several methods to manage the personal information
ecord keeping of the patient, but the process of substituting conventional paper document record
keeping system with EHR deserves special remark (Kongstvedt, 2012). This is because EHR is
oth effective and efficient in respect of keeping and maintaining patient records in a safe
manner to ensure effective safeguard of the privacy of the patients.
Purpose
The main purpose of this paper is associated with advocating the substitution of
traditional paper record keeping with the EHR systems that are more practical, and safe, as well
as very interactive for professionals while patient care (Kongstvedt, 2012). Further, this paper
would also evaluate and discuss why EHR system is beneficial than the conventional paper

ecord keeping systems, and how this can be installed and implemented properly, as well as how
healthcare organizations can also draw the benefits of resorting to EHR system while giving
patient care.
Questions
The problem and purpose of this paper have established a trust on the EHR system to
implement in the healthcare organization for better performance of patient care with safe and
secure ways (Bosek & Savage, 2007). In this consideration, there are some questions that can
influence to believe over EHR system and implement in the healthcare institutions to increase
efficiency in patient care. These questions can be seen as below:
 Which particular population is going to be benefitted by the implementation of EHR
system?
 How can the healthcare institutions install and implement this system in order to
improve the process of medical record keeping of the patients?
 How well does EHR system perform in comparison with conventional paper record
keeping systems?
 How can the efficacy and effectiveness of this system be measured by both healthcare
institutions and the selected patient population?
 How much time is needed by a healthcare organization in order to install and
implement an effective and functional EHR system?
Literature Review
According to the Jensen et al. (2012), EHR generally holds potential benefits through
establishing newer principles towards patient-stratification effectively and efficiently.
Additionally, they also stated that EHR system is beneficial to record genetic data of the patient

that provides a good understanding pertaining to genotype / phenotype relationships in case of
esearch study. The implementation of an EHR system in healthcare organization is also
supported by Hripcsak & Albers (2013) stating that the adoption of EHR can offer
unprecedented levels of data accessibility towards clinical research. However, this can be
possible after the legal authority of the management system of the organization
At the same time, Beasley & Sinsky (2014) has undertaken a critical overview about the
use of EHR system across both medicine and the Information Technology industry. Both
industries can move towards an EHR based record keeping to provide effective health care to the
patients. An EHR system is highly essential in the organizations to be robust safeguarding
against unethical and security threats. On the other side, in the thought of Sittig & Singh (2012),
EHR system usually helps to create national level strategy through helping the government to
address various safety related issues of the citizen. With the help of this system, the government
can address the preventable forms of risks that usually hamper the endeavors for creating safer
EHR. According to the Fernandez-Aleman et al. (2013), EHR system is essential in privacy and
security of the personal information of the patients. This system can improve efficiency in health
care services with cost effective manner.
