Oil and gas industry, like many other industries is vulnerable to various on shore and off shore 
accidents. Many a times, there is the need to keep into account the occupational and system safety 
requirements. Management is usually responsible for many of these accidents pertaining to 
individual mistakes or mechanical failure. To illustrate this, we need to analyze the case of Exxon 
Valdez, the Exxon Valdez was under the command of Third Mate Gregory Cousins, who was not

employee side. Analyzing the accident in detail, one comes across a few factors which were also 
involved that might have prevailed, some problems might have resulted from cutting safety corners, 
cutting a few additional features from the vessel and time to time maintenance was lacking 
certainly. Ethical risk management is vital for combatting these accidents on a larger scale. This 
includes building up adequate and efficient hazard analysis and design for safety procedures and 
equipment.
