Task summary:
· You will be required to conduct your own research to complete this task.
· Students need to submit all required templates in this assessment task. (14 example templates annotated, and changes marked up and 7 final templates
· Students must answer the questions by typing their answers into a submittable document (.doc, .docx, .pdf) or by writing in the space provided.

· Their answers to each question.
You are required to locate examples of risk assessment and hazard identification templates and create your own. You will audit the templates you locate and discuss the most useful sections and sections that you would improve or remove from the documentation you have located.
You will complete a final portfolio document of templates that can be used in a personal services business to assess risk and identify hazards.
Your portfolio submission must contain 2 x examples – annotated, changes marked up and discussed and 1 final document that you have created for each of the following:
· WHS staff training plans
· hazard identification and reporting tools
· risk assessment and control tools based on the hierarchy of control method
· emergency evacuation plans
· dangerous incident reports
· accident and injury reports
· work health and safety record keeping systems
Prepare your templates for either an actual business or a simulated business. Make sure the forms are specific to the business and the business needs, Each developed form, must include a business logo and version control. Co
ect footers and headers. A list of staff involved must be included for each form, including number of staff and their roles and responsibilities in relation to WHS.

You will be required to conduct your own research for this assessment task. You will find the websites and guides used in Assessment Task 1 helpful in beginning your research. You may seek clarification on a question from your trainer if needed. All the best!
