What have you learned about your community partner site and health disparities? 
The community part site is located in the rural areas serving the local community members. 
There are various primary based facilities for the people but many drawbacks in the services 
are also revealed. These are as illustrated. First issue is the higher incidence and higher 
disability with increased mortality rates. It is identified that higher incidence rate is a health 
disparity and along with that, the rural health community centre includes the geographic 
isolation. Geographic isolation affects access of community centre facility. There are higher 
rates of health risk behaviours and lower socioeconomic status that prevent effective caring 
for the people. The numbers of health care professionals in the rural health care are 
proportionately lower than the urban area. All these factors suggest greater disparities in 
health care facility service.  
i. What prevention services that, if available, would improve the health of the population served 
by your community organization? 
The preventable services that can be used to eliminate the disparities are mentioned. First 
service from the community is to make the health care facility accessible for each person. It 
should run various programs for different communities particularly for minority communities 
to address their specific needs. There are issues of poor ratio of health care professionals and 
for that; numbers of health care professionals should be increased.
