Background
In this assignment we have a research and development project. Given case study’s company Victaulic had a tragic accident, one of the ill-fated employee was severely injured and after few days lost his life.   Manufacturing company accepted that the cause for the accident was the faulty product. The money of the insurance is not personal for the company. In the current case study, it shows the manufacturing defects can be detected and handled by the company. This is considered as the classical philosophical debate for making the commercial expedient choice. 
The aim of this assignment is to develop and produce cause of the accident, analyze and recommendation. The relevant objectives of this assignment are as follows:
(a)	Identify an assignment related to issue as in manufacturing defects.
(b)	Provide research with data analysis and the recommendations.
Problem research
The company developed mechanical pipe-fitting for the cooling and heating system. It is contracted with the overseas contractors for construction of other products. It is not noticed by Victaulic and it in the form of substitute cheaper materials were used and it has the risk of failure under high pressure. One of the important products has cracked in the heating system that was being tested. This has resulted in severe damage to worker and the worker died some days later.
In October 2007, company shipped an order of the said product to be used in construction of the boilers in the mechanical room of the Gaylord Hotel in National Harbor Maryland . These were properly installed, but at 6:30 AM on November 21, 2007 the end cap on one of the suction diffusers failed. A worker was installing pipe insulation, and was in the process of going through a series of tests to initialize the system while it was charged and pressurized. These tests were standard procedure. During the tests, however, the end cap on one product unit cracks.
