Task Summary
In this individual task, you are expected to develop a plan for an information sheet on a medical
condition for clinicians (Nurses, GPs, specialists or other allied health professionals) to be submitted
in Assessment 2 Part B. The length of the plan is 500 words (+/-10%), excluding the reference list.

Context
Being a nurse, you will communicate with clinicians (Nurses, GPs, specialists or other allied health
professionals) about your patient’s medical condition(s) on a regular basis. This assessment task will
help you to gain skills about what types of information you should include in an information sheet
while communicating with a clinician.
This assessment aims to
ing together knowledge about individual body systems and how various
conditions can impact multiple bodily systems, with signs and symptoms seen throughout the various
systems, investigations and treatment.
Please note this assessment task (2A) is a preparatory task for Assessment 2B, where you will prepare
an information sheet on a medical condition for clinicians.
Task Instructions
To complete this assessment task, your plan must follow these steps:
1) Choose any communicable or non-communicable disease.
2) Research this condition and the bodily systems that it impacts.
3) Outline a plan for your information sheet.
• Your plan for the information sheet should include information about:
(a) Burden of disease,
(b) Signs and symptoms,
(c) Information on investigations and treatments based on the latest scientific
evidence.
(i) You can also include any screening tests and links to other helpful resources.
4) Describe the pathophysiological process and identify medical terminologies/ definitions
(where appropriate) that you will require for your information sheet.
Please also note that
• You are required to present your own original work using multiple academic references from
academic books, journals and other credible sources.
• You should present your work as double spaced text. Dot point entries and pictures can be
used.
• Academic references are to be included on a separate page using APA (6th ed.) guidelines.
