Rationale:
The purpose of this assessment task is to assess the student nurses ability to think
critically and apply the clinical reasoning cycle to plan patient care.
As a Registered Nurse, you will be required to make judgments and prioritise your
patients care multiple times per shift. This assessment task is designed to develop
the sophisticated thinking abilities that are essential to prioritising, planning and
providing safe and effective patient centred, nursing care.

In this Clinical Case Study, you have been allocated, depending on your cohort,
one (1) of the nursing problems provided below. You are to discuss its relevance,
then plan and evaluate patient centred, nursing care. The nursing problems all
elate to Da
en Roberts’ care in the first 4 hours following his discharge from
PARU and return to the ward following his surgery.

1. Potential for respiratory depression related to anaesthesia and opioid
medications and would be evidenced by respiratory rate <10Rpm,

Nursing plays an important role in patient care and it becomes more significant in the post-operative situation. The nursing problem that is significant in the scenario of Da
en Roberts 50-year-old male who has just had an Open Cholecystectomy. The patient has a history of hypertension, hyperlipidemia, and GORD. After the surgery, the saturated oxygen level was less than 95% and the respiratory rate was 10Rpm. Nursing intervention at this stage is an important context. Cholecystectomy is a procedure in which the gallbladder is removed and after the surgery, it is effective to be under professional nursing guidelines. The post-operative intervention by the nurses is a sign of effective nursing care and monitor the patients.

en Roberts
The particular risk that Da
en Robert is facing following his surgery is respiratory depression which has a connection to anesthesia and opioid medications. It is evident from his respiratory rate which is less than 10 Rpm and his saturated level of oxygen was less than 95% on RA with shallow
eathing. The other particular risks that are potential are the aspects of pain because of surgical incision more pronounced due to grimacing and a subjective pain score of 3/10 using the Numeric Pain Rating Scale. This seems to be a particular risk for Da
en during the first 4 hours of his return from the surgical ward because the effect of anesthesia that persists among the patients and it may lead to the seriousness of hypoxemia. Hypoxemia is a complication that can arise immediately in the post-operative period when the pulse oximetry becomes as low as 90% and the PO2 becomes less than 60 mmHg per ABG. It is more of a risk because it may result in hypoventilation which is a general cause and related to opioids which cause respiratory center depression and then after effect of the general anesthesia. Nursing literature has contemplated that when post-operative patients are monitored it is significant that the patient must be monitored in terms of various signs of hemo
hage, shock, sepsis, and the effects of analgesia and anesthetic. It is therefore expected that the patient’s pain must be controlled and managed well with intravenous opioids as mentioned by the various nursing literature. Studies have also contemplated ensuring that the patient has liquid analgesia and must be communicated and cooperated with the clinical staff during the post-operative period. Literature has also established certain rules and aspects that nurses must consider when they are monitoring a post-operative patient. Clinical papers have contemplated that it is effective to monitor the oxygen saturation and search that the oxygen saturation must be above 95% on air.
