Identification of Clinical Issue Selected
The clinical issue selected for the study is medication errors. As mentioned by Asensi-Vicente, Jiménez-Ruiz and Vizcaya-Moreno (2018), occurrence of medication errors in healthcare industry is very common. This clinical issue is very dangerous as it is responsible for the death of many people. It has been seen that the rate of medication errors in the healthcare industry is 42%. Most of the cases of medication errors remain unnoticed, as management do not registered them. The cases, in which either patient’s condition become severe or become reason for the death of the people. 
As suggested by Kosari (2018), the type of medication errors, which are common in healthcare industry include delaying of the medicine, overdose of the medicine and administration of the medicine in wrong form as well as prescription of the wrong medicines. It has been seen that medication errors occur due to the negligence of the care provider. The rate of the medication errors acts as the indicator of the quality of the services provided to the patients.
