Assignment Practice Scenario

Your Chief Nurse Executive (CNE) recognizes you are pursuing your BSN and has asked you to be a member of the nursing evidence-based practice (NEBP) committee. She has advised the members that the purpose is to conduct nursing research studies involving clinical practice problems. Increasing nurses' awareness of quality and safety concerns will help develop an attitude of inquiry and asking questions (Sherwood & Barnsteiner, XXXXXXXXXXBaccalaureate nurses "collaborate in problem identification, participate in the process of search, retrieval, and documentation of evidence, and integrate evidence using credible databases and internet resources" (AACN, 2008, pp. 16-17).

The CNE has advised all members of the following areas ofqualityandsafetyclinical practice that each committee member needs to consider and chooseonly oneas a priority area for conducting a research study. Sources of research problems come from nursing clinical practice (Houser, 2018):

QualitySafety
  • Patient satisfaction
  • Patient education
  • Falls
  • Non-pharmacological pain management
Errors. Examples include:
  • Medication
  • Patient identification
  • Communication
  • Readmissions
  • Length of stay
  • Costs
Healthcare Associated Infections (HAIs).
Examples include:
  • Central Line Associated Blood Stream Infections (CLABSI)
  • Catheter Associated Urinary Tract Infections (CAUTI)
  • Ventilator Associated Pneumonia (VAP)
  • Surgical Site Infections (SSI)
  • Sepsis (Screening or Prevention)
  • Discharge Teaching/Coaching
  • Transitions of Care/Level of Care
  • Pressure Ulcers
  • Skin Integrity

Your CNE has requested that you selectONLYone topic areaand complete the PPE Worksheet using onlyonetopic area. The committee will collaborate and decide as a team which nursing clinical problem area will be the priority focus of their next research study. Thank you for your valuable contributions to this important NEBP committee.

PICOT FOR NURSING PROBLEM
Student’s Name:
Student’s ID:
Introduction
Patients with diabetes are often found to get hospitalized due to hyperglycemia. Insulin is injected and oral Anti-diabetic medications are also prescribed to them. Diabetes gets controlled over these therapeutic interventions. However, nursing problems arise for most of the patients after hospital discharge and re-hospitalization is caused to many of them. The major concerns related to nursing have been pressure ulcers, hypoglycemia, blood glucose monitoring, the capability of the patient to manage their glycemia and others.
PICOT worksheet
    P (Population)
In patients with long-term diabetes often get hospitalized for an uncontrolled blood glucose level. Hyperglycemia needs to be controlled by IV insulin and other oral anti-diabetic medications.
    I (Intervention)
Peripheral artery disease (PAD) is common among diabetic patients. PAD often causes a lack of sensation leading to pressure ulcer formation. Pressure ulcers are common in the back portion or even diabetic foot ulcer is also found in many of them.
    C (Comparison)
Pressure ulcer formation can be lowered down or decreased if the proper dressing is done to the wound region. Patients who obtain a good dressing with the removal of necrotic tissues get healed in better ways compared to the patients who have to re-admit for pressure ulcer development.
    O (Outcomes)
Pressure ulcers can go back to lower stages or get decreased upon good dressing. It minimizes the mo
idity and mortality rate of the patients.
    T (Time frame in months)
    3 months.
Explanation
Pressure ulcer development has become a leading cause of mo
idity and mortality among patients with diabetes. Diabetes or hyperglycemia often develops peripheral arterial disease (PAD) or peripheral neuropathy among the patients. The patients tend to lose sensation in a particular area of the legs either in toe fingers, or palette, or lower limb (Bamba et al., 2017). Loss of sensation causes an imbalance in pressure distribution on the whole limb that ultimately results in pressure development on some particular points. The gradual elevation of the pressure points leads to the development of...
