Introduction
Safety is one of the most sensitive concept when comes to healthcare. There can be a number of areas where safety procedures have to be implied such as sanitation, surgeries and even mediations. Emergency department in a hospital can be considered as one of the most fragile departments when it comes to patient safety and this is majorly because of lack of time and information (Alomari et al. 2018). This report focuses towards one such safety concern in hospitals that is medication safety in emergency departments. Medication e
ors, checks, patient safety will be some concepts, which will be discussed along with the consequences that may arise and solutions to them.
Emergency Department and Medication Safety
Medication e
ors are one of the most commonly occu
ing medical e
ors in hospitalized patients. Benjamin et al. (2018) mentioned that medication e
or rate is 3 times in emergency cases or Emergency Departments (ED) than in any other department. The emergency care or the emergency department can be considered as one of the most high-risk environment for medication e
ors to occur. This is majorly because the cases or patients in emergency care may have critical condition and therefore, very less time is available for the healthcare provider to make a decision.
Some of the commonly known rationales responsible for high rate of medications e
ors are due to very less information regarding the patient because patient can be medically complex (Smeulers et al. 2015). Moreover, there is a lack of standard drug dosing and formulations or weight-based dosing. Further, emergency departments have a hectic environment with frequent inte
uptions, which leads to difficulty in decision-making. Further, Weant, Bailey and Baker (2014) mentioned that medication e
or rates in emergency departments of hospitals could be as high as 4-14% normally however; it can be even 39% in paediatric emergency departments. Most of these ware related to the lack of Emergency Physician (EP) which are to be required in ED.
Patient Safety
Patient safety in ED is one of the major concern and ED lacks time period required for the decision making process. This is the major factor identified behind high e
or rate. The NSW guidelines identify three basic e
or types, which leads to mediation e
or. It is not always the medicine type, which can cause e
or instead the dosage, and knowledge of dosage can be another factor that can lead to e
ors (Muscillo, 2018).
NSW identifies that e
or can occur when —
· slips or lapses occur,
· Lack of attention takes place, due to which staff members fails to notice potential problem, which needed reporting
· The professionals do not having knowledge regarding the actions to be performed
However, when it comes to medication e
ors first and second points are major reasons. Staff members may be held responsible for the occu
ence of e
ors due to the above-mentioned rationales. Other than this, medicines with similar names or packaging, medications that are not commonly prescribed medicines that may be allergic to patient and no knowledge of therapeutic levels are some another common reasons which leads to a compromise in patient’s safety.
