Instructions: Choose ONE of the three case studies provided.
Carefully read all materials provided for your chosen case study.
Use the Word template provided to complete your essay on your chosen
case study. You will need to research the topic beyond your lecture and
workshop material. It is highly recommended that you start with the
textbook.
Include in text citations in your essay and a reference list at the end.

Case Study: Stuart
Stuart is a 72yo male who lives alone since his partner died 2 years ago. He has come to see his GP today as
he has had several episodes where he has felt faint, and “may even have passed out on a couple of
occasions”. It is some time since Stuart attended the clinic and you note when weighing him that he has lost
a significant amount of weight, and he doesn’t present as the robust active older person that you remember.
He mentions that he doesn’t cook much and relies on the take-away shop around the corner for most of his
food these days.

He explains that he has been very lonely since he lost his partner but is excited to reveal a new zest for life
now that he has met a new friend, with whom he recently travelled to Cusco, Peru (3400 m above sea level).
While he thoroughly enjoyed the trip, he did mention he found it more difficult than normal to
eathe
there. His relationship has developed sexually, but he has had some difficulty performing. Stuart thought it
was his prostate giving him trouble, but with urging from his new partner he bought some Viagra nasal spray
over the internet.

The urinalysis shows high level of ketone, protein , glucose and leukocytes in the urine. The assessment involves detailed analysis of this case study while answering the specific questions.
Change in Volume and Pressure
As Stuart had visited a high altitude place recently, he would be facing forced inspiration and expiration to achieve inhalation at rest. At high altitudes, the atmospheric pressure is less. At altitudes above 3000 m, the person may experience hypoxia and acute mountain sickness (AMS) which includes nausea, shortness of
eath, headache and dizziness. Due to reduced arterial pressure of oxygen, the lungs show elevated heart rate, acclimatization and hyperventilation to achieve sufficient oxygen supply for the tissues. These changes occur due to increase in tidal volume due to ventilation. The lung volume reduces due to accumulation of extravascular fluid in pulmonary interstitium. At high altitude, there is low oxygen saturation level and less oxygen is available in the airflow.
The gas exchange at high altitude with little oxygen and reduced blood flow causes increased elevated ventilation and alveolar dead spaces. The diffusion of inhaled oxygen in the pulmonary capillary blood is reduced due to decreased pressure of alveolar oxygen at high altitude (Wagner, 2015). It may cause failure of equili
ium.
After consuming a bowl of pasta, the parasympathetic division of Autonomic Nervous System (ANS) will regulate the digestion. It will increase the peristalsis and enable secretion by digestive glands. Ghrelin suppression gives feedback signal to
ain for stopping eating (Guarino, Nannipieri, Iervasi, et al, 2017).This mechanism helps in reducing the calorie intake. The gastrointestinal tract also releases Peptide-YY and GLP-1 hormones in response to meal size after the meal intake. There hormones regulate satiety and induce taste aversion.
The glucagon and insulin hormones produced by pancreas would regulate the blood glucose level. The insulin would control the glucose uptake by different cells of the body and the glucagon would regulate the release of glucose from the body. The glucose homeostasis involves a balance of insulin and glucagon to maintain appropriate blood glucose levels. If the level of any one hormone is more or less, the blood sugar level may increase or decrease. The insulin helps in glucose absorption of cells, lower down the blood sugar and gives energy to the cells with glucose (Röder, Wu, Liu & Han, 2016). In case the blood sugar levels are low, the glucagon is released by pancreas. The glucagon stimulates the liver to release the excessive glucose, which elevates the blood sugar.
Role of Kidneys
Kidneys maintain fluid balance by adjusting the urine concentration as per the water needs of the body (Chapin & Hajjar, 2015). Kidneys conserve the water when the body is dehydrated and excretes excess water by making it more dilute. The ADH hormone helps in water retention of body and stimulates the reabsorption of water by kidneys (Cuzzo, Padala & Lappin, 2020). It controls the osmotic balance, kidney functioning and regulation of Blood pressure.
The urinalysis report of Stuart show positive glucose in urine. It shows the blood glucose level is very high and hence some glucose is appearing in urine. It shows Stuart is at risk of maintaining homeostatic fluid mechanics.
