Introduction Why am I doing this? A performance review (or performance evaluation, performance appraisal etc.) is a method by which an employee’s performance is documented and evaluated. It is part of a career development program and a means for supervisors to monitor, guide and reward that person’s contributions to the workplace. It is an opportunity for the staff member and the supervisor to develop an understanding of the other person’s needs and expectations and to ensure that these are aligned, not only with each other, but also with the needs and expectations of the work associated with the position. It requires honesty, clarity and, in some cases, a tough conversation.
Brief description of assessment task
This assessment task requires the student to engage in a performance discussion with their internship supervisor. The supervisor will evaluate the student’s performance and outcomes against the outcomes identified in their learning contract. Students will also be asked to prepare and provide a selfassessment against the goals and outcomes identified and reflect on their performance prior to evaluation by the supervisor.
Detail of student output
Students will self-assess their internship performance and prepare a reflective report of not longer than 1500 words with a detailed account of their accomplishments for supervisor evaluation. The self-assessment report should include evidence of accomplishments against set goals and outcomes identified in the learning contract along with a self-rating on the quality, integrity and timeliness of the work undertaken.

Students will be provided with written feedback and constructive criticism of their submission in time to influence their final portfolio submission.

Completed Supervisor evaluation and student self-assessment to be submitted within 2 weeks of completion of placement.
Your performance evaluation You have set the benchmark against which you will be evaluated in your Learning Contract. Near the end of your internship is the time to self-assess how well you have achieved your goals. Your supervisor will assess your performance against the goals set out in your learning contract during your performance evaluation in the last week of your internship. Copy each statement of objective from your learning contract to the Performance Review template. For each learning objective, provide a statement indicating how well you think you have achieved the objective based on the measures you previously identified. If you have not met your objective you need to indicate why. This could be as simple as the opportunity did not arise, or the task was harder than you had anticipated. This is an opportunity for you to remind your supervisor of your achievements during your internship, your contributions to the team/section/division/organisation. Point out the things that did not go so well but reflect on why and how you could improve. Be positive. Remember this is a learning opportunity, not just through what you completed during your time at the organisation but in the learning achieved through reflecting on your performance.

Why am I doing this? Internship is a program to increase their capability and give the
opportunity to do the practical work in a company. Internship is a procedure where student to
learn how to apply theory in a practical way. Internship means we do the work as a employee
in company for limited time. Internships are a unique and innovative approach. We connect the
world through the internship program me. I faced lots of challenges during the internship
program me and got the lots of opportunity. Internship gives the exposure for student how they
work after the study. In internship program period is six week. We gain only theoretical
