Introduction
The following assignment consists of two Booklets which is made with an intention to provide the guidance to the student and new staff of the hospital setting, which is made by the senior carer based on their experience in using person-centred practice in providing high quality care to the patient. These Booklets help them understands the principle of holistic person-centred practices. The Booklet 1 addresses the definition of the holistic person-centred care and its application. Apart from this, it provides the idea about implementing this approach in developing care plan for the patients and the challenges and dilemmas in the healthcare setting, which can affect the implication of person-centred care constantly.
On the contrary, the Booklet 2 provides the information about the importance of knowing the legislation, policies and procedures, which are included or applicable while providing person-centred care. It addresses the problems developed by following workplace policies and solutions to mitigate it.

Holistic Approach to Person-Centred Care
Holistic person-centred care can be defined as the type of high-quality care, which focuses on addressing the individual needs of the person. As mentioned by Mukwege and Berg (2016), it has been seen that this kind of approach empowers the patients to determine their need and encourages their reflection and self-awareness. This approach benefitted both service seeker and care providers as healthcare professionals, which adopt the holistic person-centred care, have both personal and professional growth and development.
In this kind of approach, first holistic assessment is done. Holistic assessment considered as the key element in fulfilling the needs of the patients. When conducting the holistic assessment, many factors are taken into consideration such as physical, social, psychological and spiritual wellbeing of the individual. The four dimensions which professionals need to consider while conducting holistic assessment includes connection, caring attitude, communication, and control.
As stated by Feo et al. (2017), communication is a key to develop the connection with the patient. When care provider must develop the therapeutic communication with the patient. This can be done when by showing compassion, caring attitude, and empathy towards the patient. Good communication helps in earning the trust, which is foundation of the successful treatment. When care providers failed to earn the trust of the patient, then the patient does not trust the healthcare setting or treatment given to them. They must give respect to the patients. Apart from this, patients must be felt in control, which means care provider must provide advocacy, rights, and responsibilities, which can help the patients to take the decisions necessary for their safe and quality treatment.
Comparison between medical and social model of person-centred practice healthcare or support services
Social model of the person-centred care can be used by social services in providing high quality care to the patient. On the contrary, medical model of person-centred care is adopted by healthcare professionals to ensure high quality treatment can be given to the person.
