Workarounds are common in everything we do, whether information gathering and use is automated or not, and outside information systems as well. A workaround is a process that people use in an attempt to simplify a task, shorten the time something takes to do, or avoid doing something the y do not like to do, are uncomfortable doing, or do not understand well enough to do correctly. A classic workaround in the EHR environment is one in which nurses circumvent the barcode medication administration record system by printing extra bar codes for patients and “reading” the bar codes at the nurses’ station so it does not have to be done at the bedside. This defeats the five-rights of medication administration.

Today, many physicians are still struggling with using the EHR at the point-of-care, especially in the office and clinic setting. They believe it extends the time it takes to complete documentation and they believe and even see their patients getting frustrated that they are constantly attending to their computer and not the patient. Workarounds for this include using a scribe, documenting after hours, using comment fields rather than structured data fields to enter notes, and giving up use of the EHR altogether.


  1. Research and identify why these workarounds are ineffective and potentially dangerous. Prepare for a role-play exercise. Consider the patient viewpoint; take into account your own experience or that of your family members. Consider the physician viewpoint and the need to see many patients in a day.
  2. For your initial post to the discussion board, you will speak in the role of the patient. Identify issues in point-of-care documentation and then find ways to overcome these issues while still using the EHR at the point of care. Please think outside of the traditional way physicians interact with health records and patients to afford even more opportunities for improvement in documentation and data usage.

ROLE PLAY:
NURSE : Doctor, shall I call the next patient?
DOCTOR : Yes please.
(The patient enters the room)
PATIENT : Good morning Doctor. I am Peter Gardener.
DOCTOR : Good morning Peter. How are you doing?
PATIENT : I am feeling uneasy since yesterday evening. I feel kind of nausea and weakness in my body, unable to do anything. I also had an episode of vomiting.
(The nurse meanwhile checks the patient’s vitals and notes them down)
NURSE : Doctor, there is a rise in the blood pressure and heartrate.
DOCTOR (looks at the computer and asks the patient) : How old are you? Have you ever experienced this condition before?
PATIENT : I am 45 years old and I never had such an experience of nausea before, though weakness is common with me in the recent times.
DOCTOR (fills the data in the computer and continues his questions) : Do you take medicines for hypertension? If yes, which one and since how long?
PATIENT : Previously when I consulted a doctor, he said me to have a check on my blood pressure levels for a week’s time and meet him again. So that he could prescribe me medications if there were abnormalities in it. But I could not follow the check-ups and missed to meet him. So, I do not take any medicines.
DOCTOR : You have an increased blood pressure levels and as you mentioned, this is not the first time. So, you will have to take medications here onwards to keep your blood pressure under control.
(Doctor rarely looks up at the patient, and keeps on typing, which makes the patient unsatisfied with his visit)
PATIENT : Doctor, is there anything else that I will have to follow?
DOCTOR : Yes, along with the medications, you will have to follow proper balanced diet and exercise...
