Workarounds are common in everything we do, whether information gathering and use is automated or not, and outside information systems as well. A workaround is a process that people use in an attempt to simplify a task, shorten the time something takes to do, or avoid doing something the y do not like to do, are uncomfortable doing, or do not understand well enough to do correctly. A classic workaround in the EHR environment is one in which nurses circumvent the barcode medication administration record system by printing extra bar codes for patients and “reading” the bar codes at the nurses’ station so it does not have to be done at the bedside. This defeats the five-rights of medication administration.
Today, many physicians are still struggling with using the EHR at the point-of-care, especially in the office and clinic setting. They believe it extends the time it takes to complete documentation and they believe and even see their patients getting frustrated that they are constantly attending to their computer and not the patient. Workarounds for this include using a scribe, documenting after hours, using comment fields rather than structured data fields to enter notes, and giving up use of the EHR altogether.
