Practice for Success
‘A Practice for success or Formative assessment has a developmental purpose and is designed to help learners learn more effectively by giving them feedback on their performance and on how it can be improved and/or maintained’ (UoS – Assessment & feedback Policy)
To provide students with support on their summative assessment, we have provided a ‘Practice for Success’ opportunity where a sample of your work (draft), based on the final assessment criteria may be submitted, and feedback will be provided. This is an optional piece of work, but we strongly encourage you to take advantage of this opportunity as it provides an excellent way to ensure you are structuring your work appropriately, addressing the brief correctly and referencing appropriately. Details of this process are highlighted below.
Practice for Success criteria
• Provide a brief outline of your chosen episode of care (approx 150 words)
• Choose ONE of the following sections of the assessment criteria to focus your ‘practice for success’ discussion on. You are free to choose whichever section you would like feedback on (approx 600 words)
o Describe the six components of ‘The Personalised care operating model’ as per the NHS long term plan (2019), in relation to the episode of care and explain the importance of partnership working between the nurse and person in achieving a joined-up approach to meeting their needs
o Explain how a nurse can use effective communication and interpersonal skills within the episode of care, in identifying what matters to the person
o Describe one advantage of and one barrier to interprofessional teamwork in relation to the persons episode of care
o Describe how social prescribing could support the persons health and wellbeing as well as the health and wellbeing of their family and carers
• Provide a full reference list for the sources of literature you have used following the current APA 7th Harvard guidelines. The reference list is NOT included in your final word count.
