Task: In this assessment students will:
1. Write a
ief explanation of what "Closing the Gap" (CTG) policy is, including the history and data that influenced the policy's creation. Then discuss the significance of the CTG policy on Aboriginal and To
es Strait Islander People's health outcomes, using the relevant literature and statistics. Please include any challenges and/or ba
iers which may have affected the changes.
2. Analyse the impact of Judy's Diabetes and, how it is addressed within the CTG, including the long term consequences to health and social determinants. Your answer should include discussion of policy.
3. Discuss the benefits of Judy being registered with CTG, and, how this may have an impact on her social determinants.
4. Define the roles for the Aboriginal Medical Service (AMS) and, Aboriginal Liaison Officer (ALO), and,

ing Judy to the community health centre, explain what services they provide and the relationship between the health systems.

ASSESSMENT 2 ADDITIONAL INFORMATION - INCLUDING SCENARIO

For the purposes of this assessment you will be expected to consider the following scenario
Framework:
You are a senior RN working at UTS Community Health Centre that does home visits.
Patient Background:
Your new patient is Judy, a 57yr old Aboriginal woman with type 2 Diabetes who has been discharged home with a leg ulcer for daily dressings. She is a widow, cu
ently living by herself in her own home. Judy has always worked part time as a primary school teacher but due to her mobility issues has now retired. She does not own a car and relies on public transport. She lives 10km from the closest health care facility. Judy has two grown up children, who live close by, and who assist when they can. They will be present for the first visit.
The refe
al is from the local AMS. The request for daily dressing was from the ALO for the community nurses to attend home visits for daily dressings to the leg ulcer as AMS are not able to attend to this need. The ALO is unable to come to the first home visit for introduction.
The accompanying discharge letter from the hospital states that she may be eligible to be registered for CTG.
Before attending your first visit, and as part of your preparation for the visit, please answer the following:
1. Write a

2. Analyse the potential long term consequences of Judy's Diabetes to 3 social determinants of health and, how they are addressed within the CTG policy. Your answer should include discussion of the CTG policy.
3. Discuss the potential benefits and impact of being registered with CTG on Judy’s social determinants of health. Please discuss this in relation the 3 social determinants of health discussed in question 2.
4. Define the roles for the Aboriginal Medical Service (AMS) and, Aboriginal Liaison Officer (ALO). Explain what services they provide.
5. Give the reasons why the AMS and ALO would be involved Judy’s care and would be refe
ing Judy to the community health centre. Describe the relationship between the AMS, ALO and community health centre.
