Aim of assessment
The purpose for this case study is to enable students to produce a nurse-led case management plan,
using evidence-based information relevant to nurses and up-to-date Australian statistics, for
improving the health and quality of life for an older adult who have suffered a myocardial infarction.
Content should include discussion on pathophysiological treatment options, based on national
guidelines and implications for nursing practice.
Patient profile
Kath Ha
is is a 65-year old lady who lives alone. Kath was doing light cleaning around the house,
prior to the onset of her pain. Mrs. Ha
is has a history of unstable angina, hypertension and
diabetes mellitus type 2. She is a cigarette smoker, smoking two packets of cigarettes a day, for
approximately 40 years. A recent history of indigestion suggests an active peptic ulcer. She is also
obese (her body mass index is 32). She has had a history of head trauma due to an incident a few
months ago, where she fell down a set of stairs. Mrs Ha
is’ cu
ent medication includes; GTN,
carvedilol, and metformin.
Subjective data
• She had taken three glyceryl trinitrate tablets, which had little effect on reducing her chest
pain.

Your assignment is to produce a written piece of work addressing the following areas;

PATHOPHYSIOLOGY
ï‚· Briefly explain the pathophysiology of myocardial infarction in this patient. In your answer
identify the clinical manifestations of an MI demonstrated by Kath Ha
is and why these are
occu
ing. Identify the risk factors evident within this case study. How would you manage
Mrs Ha
is’ care?

MANAGEMENT
ï‚· Outline the immediate nursing and medical management Mrs Ha
is requires in the
treatment of an acute MI. In your answer include the pharmacological management
equired by a patient with an acute coronary syndrome.

MEDICATIONS
ï‚· Identify three medications that would be used in the treatment of an MI and outline the
indication for use, mechanism of action, administration method, side effects, and nursing
care required for these medications (No tables).

Important details about your assessment
ï‚· This essay requires a standard essay introduction and conclusion.
ï‚· There is a word limit of 1500 words (+/- 10% as per SNM Presentation and Assignment
Guidelines).
ï‚· Marks will be allocated as per SNM Assessment Ru
ic & the essay specific criteria below.
 Submission instructions and the link is accessible under the ‘assessments’ tab.
ï‚· Students should refer to the SNM Presentation and Assignment Guide, ECU APA
Referencing Guide, and the SNM Marking Assessment Ru
ic.
ï‚· Graphs, tables and flow charts to support the statistical data will not be accepted.
Marking criteria:

Below you will find two tables: The first is the SNM generic marking ru
ic, the second is an
interpretation of that ru
ic to help you recognise which areas are important (it shows you what
elements you need to address in order to get full marks). The points in
ackets show you how those
elements are weighted, e.g. the introduction can get up to 2 points etc. Elements with more points
would be expected to require more words/paragraphs.

Introduction
The cu
ent study is a case study of Mrs Ha
is, suffering from symptoms of myocardial infarction. In this cu
ent analysis, the risk factors, the medication, the clinical interventions will be assessed to understand how Mrs Ha
is can be assisted so that her MI condition can be controlled. The various clinical manifestations and the diagnosis will be evaluated for treatment planning.
Pathophysiology
The case study Kath Ha
is reflects some of the pathophysiologies that highlight symptoms of Myocardial Infarction. The first indication is her unstable angina. The unstable angina condition of Mrs Ha
is highlights the possible blockage of the coronary artery. In most cases, unstable angina results in the myocardial infarction on the basis of the intensity of blockage and location of the blockage. Further, unstable angina also signifies an intermediate state leading to MI. The second symptom is her hypertension (Heusch & Gersh, 2017). Hypertension condition is an age-oriented risk that can results in mortality due to the ischemic heart condition. Further hypertension will have an adverse impact on the cardiac outcome resulting in the acute MI condition. In addition, Mrs Ha
is’ type 2 diabetic condition will result in the higher outcome of MI condition compared to the other non-diabetic patient.
Further, Mrs Ha
is' electrolyte level shows a Sodium 138 level. This shows that in many of the elderly patient in the case are observed to have
=138 mEq/L then there is a chance that there is an existence of Hyponatremia. Further, her level of Hba1c emphasizes on her T2D condition with very poor control of glucose level in her blood (Hopstock et al., 2018). Some of the clinical manifestations that are showed by Mrs Ha
is, reflecting on MI condition includes nausea associated due to the inferior myocardial wall during infarction, distress with chest pain. In addition, other condition includes pale skin, problems with ingestion with peptic ulcer. The Active Peptic Ulcer condition is probably due to increased vascular insufficiency in the intestinal wall. Some of the risk factors that are highlighted by the case of Mrs Ha
is insisted that the MI condition are hypertension, Type 2 diabetes, and abdominal obesity. In addition, Mrs Ha
is had been smoking for the last 40 years that adds to the risk factor for MI (Be
y et al., 2017). Additionally, the patient also complains of pain. Cu
ently, Mrs Ha
is is taking a drug combination of GTN, carvedilol, and metformin which helps her maintain glycerol control.
For the management of Mrs Ha
is's condition, there are four levels of interventions that can be taken. Firstly, she needs to be educated on her diabetic condition so that she can be provided with treatment options, exercise plan to reduce her obesity and blood glucose monitoring as all these aspects will contribute in the management of MI condition. Additionally, Mrs Ha
is needs to be provided with nutritional therapy which will include the basic kilojoule of energy she requires to consumer every. This will include 50% of ca
ohydrate, 20% of protein and 10% of fatty acids. Further, she needs to exercise and overcome her sedentary lifestyle. Intake of hypoglycemic agent orally that will keep her glycemic condition in control.
Management
The therapy for MI and management may include thrombolytic therapy. In addition, it is important that there is certain lifestyle modification for Mrs Ha
is. This will help in...
