This information which is in the Student Assessment pack is to be handed to each student to outline the assessment requirements
Student Instructions
This assessment is to be undertaken in a fully equipped aged care facility.
For this assessment, you are to be observed on 3 separate occasions (2 observations by your workplace supervisor and 1 observation by your assessor). For this assessment, you will need to communicate and work in health or community services in a workplace.
1. While you are communicating and working in health or community services, you will need to ensure that you demonstrate the following:
a. Effective communication skills in 3 different work situations
. Clarify workplace instructions with appropriate people
c. Negotiate timeframes with 2 colleagues around work tasks
d. Respond appropriately to 3 different situations where communication constraints are present
e. Complete 2 written or electronic workplace documents to organisation standards
Your 3 observations will be observed as follows:
· 2 observations by your host employer / workplace supervisor that will be verified by your assessor from your College
· 1 observation by an Assessor from your College

Student Instructions
This is an open book written test. Therefore, you are allowed to

Your assessor will set a date and time for this open book written test.
On the test date:
1. Your assessor will set ground rules including no phones, talking, etc.
2. Your assessor will pass a copy of the assessment to you.
3. Your assessor will allow time for reading and questions before commencing assessment.

This assessment is to be undertaken in a fully equipped aged care facility.

You will need to perform the following at your workplace:

1. Think about a time you have followed safe work practices for direct client care. Describe the situation, discuss the safe work practices you followed throughout the job, discuss any hazards or risks you identified, the steps you took to remove or minimize the hazard, and the reporting procedures you followed or would have followed to report hazards, incidents, injuries or client-related risk factors.
    I was working in the healthcare environment there were a lot of chances of facing a hazard or risk and I witnessed it when I was working as a registered nurse in a hospital (Australia). However, I provided care to a bedridden patient namely Stacy. She had right-side paralysis so she was unable to move so as a registered nurse I provided daily care like personal care, nasogastric feed, and cleaning. These were all of the activities that exposed me to several hazards consisting of infectious agents. I identified that there was a bedsore due to the infection and lack of blood circulation on her back. After seeing the Nursing Care Plan I informed my supervisor and filled an incident form immediately. I did the documentation and after that, the head nurse was informed by the supervisor for changing the patient’s position every two hours and also provide back care twice a day and put it into the Nursing Care Plan.
The safe work practices that I followed throughout my job:
· Following the workplace policies for minimizing the risk
· Following the workplace policies for a safe workplace
· Reporting them to the designated person
· Identifying incidents and injuries to the designated person
· Identifying the client risk factors
