Purpose of the assessment
As you progress through your nursing career you will identify areas where more education or health promotion is required to change behaviours. This assignment provides you with knowledge of the skills and processes used in preparing health education or health promotion resources. Importantly it encourages you to identify that any form of health education or promotion should be designed to meet the needs of the target group and involve some form of community consultation to ascertain the need and acceptance of the health promotion. This assessment provides opportunity to develop your own health promotion project.

Your health promotion plan should address the following criteria:
1. A description of your specific target group and community (for example: adult males indigenous and/or non-indigenous who are aged 40 years with a diagnosis of COPD or middle-aged and older people 45 or older, have a family history of diabetes, or are overweight or obese). This contributes to making your health promotion innovative and focussed.
2. A detailed outline of why this health promotion is beneficial for your target group. For example, how you anticipate it will improve health outcomes for the chosen target group/community. Include health data/statistics to back up this as a health concern, such as burden of disease.
3. A description of what your goals/ aims for your health promotion are and what the health promotion project is aiming to achieve (review the SMART mnemonic on page 215 of the set text Fleming & Parker, 2015 Introduction to Public Health, 3E).
4. You should identify which of the Health Promotion Management Perspectives this falls into and
iefly outline why it falls into this category (Health Promotion Management Perspectives are classified as primary, secondary or tertiary- to find out more about this refer to the attached (page 316 of the set text Fleming & Parker, 2015 Introduction to Public Health, 3E)
5. Identify who the additional stakeholders will be for the project, and what community consultation will take place. Include specific information on who this health promotion project will need to include and why.
6. Present a specific and original health message/ logo that you have developed to suit your health promotion (for example: Your smoking harms others).  Make sure this would be appropriate for your target group.
7. Provide a detailed outline of what the health promotion activity will entail and how you will promote this to the target group. Look at other health promotion activities for ideas such as the Breast Cancer Mother’s Day Classic Fun Run or the posters you see around the community.  Consider what sort of resources will work to get the message out to your target group such as newspapers, sports event, school visit, radio- what will work for your target audience)
8. An outline of how you plan to evaluate the success of your health promotion.  Evaluation is an important step, research to find the best way to assess if your health promotion would be successful in achieving its aims/goals.

Introduction
    COPD or Chronic Obstructive Pulmonary Disease is one of the chronic health problems, which have affected the Australian Aboriginal and To
es Strait Islander population more than the normal population. It is a major cause of mo
idity and it has substantially increased the mortality rate in the Australian population as a whole. As supported by Heraganahally et al. (2019), it is the third most common cause of death worldwide.
1. Description of Specific Target Group and Community
    The Aboriginal and To
es Strait Islander population in Australia are Indigenous peoples of Australia. As supported by Liberman (2017), they include hundreds of different types of groups within them who possess their own distinct set of languages, histories and cultural traditions amongst them that is widely distinct from their non-Indigenous fellow citizens. They represent around 3.3% of the total Australian population consisting of 91% of the Aboriginal origin, 5% of the To
es Strait Islander origin and 4% of them being both Aboriginal and To
es Strait Islander origin population
Although prone to living in an u
an area, these populations have shown the prevalence of being lower in socioeconomic status than the normal populations. The Aboriginal and To
es Strait Islander population in Australia have shown significantly lesser health outcomes when related to their non-Indigenous fellow citizens, which make them liable to experience a variety of health related issues at a much earlier age and substantially lower life expectancy than others (She
iff et al., 2019). This population also lags behind the other citizens of Australia in several other healthcare related issues as an outcome of being a low-socioeconomic group and their sedentary and unhealthy lifestyles.
According to the prevalence of COPD increases rapidly with age, which results in the more common occu
ence of COPD diagnosis in the elderly population of 45 years or older having a family history of diabetes, smoking habit or are overweight or obese. Regarding the same, there is 81% of the total Aboriginal population affected by obesity in Australia, as supported by She
iff et al. (2019). There is an urgent need to address the issue regarding the healthcare planning promotion amongst the targeted group, which is the elderly male group of population of the Aboriginal and To
es Strait Islander people in Australia and project regarding proper education on risk factors, awareness and prevention of COPD is required to be given to the targeted population.
2. Benefits of Executing this Health Promotion for Target Group
A statistical study done by Heraganahally et al. (2019), stated that out of the 767 patients of Aboriginal population who came to the hospital, 49% (n=380) had a clinical diagnosis of COPD. A number of characteristics such as the smoking status, respiratory symptoms and demographic features were taken into account in this statistical study. Also during the study period, there was a death of 30 patients diagnosed with COPD were noted to have died, representing a mortality rate of 9% amongst them.
Another statistical study of the target group was done by Blackstock, et al. (2018) and the results showed that out of the total, about 32% of Aboriginal Australians were diagnosed with co‐existent
onchiectasis with COPD. They were having prolific cough, repeated hospital admission and slightly there was a noticeable difference observed in the functioning of their lungs in patients having both COPD and
onchiectasis.
The average age of the people diagnosed with COPD was between 56 to 59 years respectively among the patients, out of which 57 % were males. Hence, health promotion is beneficial for the target group since the prevalence of smoking is more in the males, especially the elderly people out of work stress or any other factors than in the females and thus the burden of the disease would be more amongst them.
