	Despite the grave consequences of adverse events in healthcare organizations in USA, the occurrence rates are very high. due to lack of proper training, lack of communication, lack of idea about medication standards and failure of care services surveillance are among the major causes that creates adverse events, often leading to serious health issues and even death of care seekers (INSTITUTE OF MEDICINE, 1999). 
Impact of Adverse Event on Healthcare Organization
The occurrence of adverse events, such as erroneous blood transfusions to care seekers, is a result of personal level human errors. However, it affects the perspectives of all the service users of the healthcare organization negatively. An error, occurring at the time of blood transfusion, points at the lack of proper training of the care staffs and management, who are actually responsible of retaining the standards of care services. The error of blood transfusion, as identified by Smith (2018), generally occurs in the form of improper noting of patients’ blood group, existence of blood allergies and the necessity of blood transfusion altogether.
