In pathophysiology deep vein thrombosis is the potential problem which patient is likely to have due to reduced mobility. It is not the actual problem so it has to be highlighted accordingly. But sepsis is the actual problem as patient have it due to infection.

Instead of saying “acceptable BP” as an outcome, identify what range you want the BP to be in for your patient.

Introduction
This Nursing Care Plan report discusses the two chosen problems such as firstly, comes to the aspect of the actual problem i.e. sepsis related to the infection and then comes to the aspect of potential problem i.e. deep vein thrombosis related to the reduced mobility experienced by Ms. Alice McCallum, 74/F. Other than this, this report will highlight the pathophysiology of these two problems, symptoms and then will provide certain detail about the care plan that the concerned nurse is going to implement in order to treat or handle the concerned patient in an effective manner.
Pathophysiology of Sepsis
It is an evident fact and needs to be mentioned in this report as noticed by the concerned nurse that the patient is suffering from sepsis.
Sepsis is a common cause of illness and death in today’s time. It is defined as the extreme response to infection when the microorganisms attack the body. Sepsis starts as an infection in which the microorganisms like bacteria, virus, fungi, etc. invade the body. Sepsis can cause acute organ dysfunction trailed by multi-organ failure and death. In the early stages of sepsis, it is considered by the occu
ence of two or more of the following features: “Heart rate > 90/min, Temperature <36°C or >38°C, Respiratory rate > 20/min or PaCO2 <4.3kPa, White cell count > 12 x 109/l changed mental status, blood glucose>7.7mmol/l in absence of diabetes” as explained by (Mira, 2017).
Whenever any foreign microorganisms attack the body, the body reacts by activating its immune system. The immune system releases certain cells that fight with the microorganisms. In sepsis, there is an extravagant flow of these immune cells in the blood vessels that makes it a life-threatening condition. Gotts (2016) described the pathophysiology of sepsis in his paper as well as its clinical management. He mentioned that the vulnerable group of population i.e. people above 65 years of age, co-mo
id conditions like diabetes and high blood pressure, immunosuppression, etc. are at higher risk of having sepsis. The endothelium is the considered an organ that covers a large area on the body. It performs a variety of functions like maintaining the vasomotor tone, regulation of substances moving to and fro from the tissue, the coagulation system of the body and it also maintains the balance between starting and ending inflammation. In the condition of sepsis, endothelium loses these functions leading to edema of tissues all over the body. In the circulatory system, the blood vessels get clogged due to a large number of RBC and WBC. The accumulation of high protein fluid in the lungs leads to reduced lung function in sepsis which is the starting point of multi organ failure (Taeb, 2017).
In the case of Ms. Alice, the high temperature, decreased blood pressure, increased heart rate, and high values of urea and creatinine are indicative of sepsis.
Pathophysiology of Deep Vein Thrombosis
Ms. Alice has potential to develop Deep Vein Thrombosis (DVT) due to the reduced mobility. DVT refers to a condition when a blood clot or thrombus develops in one or more deep veins of the body. The main symptom of DVT is leg swelling and pain but it may be asymptomatic is some cases. The two most common reasons of DVT are a problem in the clotting mechanism of the body or immobility due to health, accident or surgery (Di Nisio 2016). DVT can become very serious if the clot moves to the veins of the lungs which may lead to blockage in blood flow and the condition is known as pulmonary embolism. The most common signs and symptoms of DVT are pain, swelling, warmth and redness in the affected leg or sometimes in both the legs. The situation would become dangerous if there is pulmonary embolism and thus DVT should be taken care of as soon as possible and pulmonary embolism should be prevented.
DVT may be caused by any things that interferes with the normal circulation and clotting mechanism of the blood in the body. This may happen due to any injury directly to the vein due to an accident, surgery, medications or lack of movement. There are many risk factors for DVT and the ones that might cause DVT in Alice are age, prolonged bed rest and her condition of sepsis (Visweshwar, 2017).
Alice is having frequent nausea and vomiting, secondly she is also facing the issue of confusion which is one of the symptom for the deep vein thrombosis. Alice also loss of energy and drowsiness which in turn is making the health condition worse. She also has muscular spasms and weakness and that is one of the vital symptom of the Deep vein thrombosis. Alice is also restless and i
itable which is an extreme symptom of the Deep vein thrombosis. In addition to this the diagnoses also revealed that the Sodium content in the body gets diluted and which is dangerous in Deep Vein Thrombosis (Lin, 2020).
