Partnering with Consumers
Student Resource
The below information aims to add some further depth and detail to that provided in the unit outline as well as a
ief overview of the expectations and how best to prepare this assessment task . A number of hyperlinks have been included which further add to this resource and information useful to successfully complete this assessment task.
What is the aim of this assessment item?
The aim of this assessment task is to essentially critique a resource intended to promote/maintain person - centred care in your organization and relate it to a specific action under Standard 2:: Partnering with Consumers from the National safety and quality health Service Standards .
How do I best prepare to undertake this assessment task?
Please ensure you have accessed the resources associated with Module 2 in this unit as this contains some of the theoretical background to person - centred nursing and associated framework s that forms the basis of this assessment task.
Additionally, you will need to re - familiarise yourself with Standard 2, Partnering with Consumers
from the Australian Commission on Safety and Quality in Health Care, The
Fact sheet
provided on this site provides a use full overview and starting point for your understanding.
I think I may be undertaking PEP in a health organisation where it may not be mandatory to be accredited against the National Safety and Quality Health Service Standards .
The Standards exist primarily to both protect the public from harm, and to improve quality assurance processe. As such, they are deemed by health departments as the as the mandated accreditation tool for Hospitals, Day - procedure services and Multipurpose services/small hospitals (AACSQHC, XXXXXXXXXXOther private health facilities (such as aged care facilities, private hospitals, etc)) can opt to use the standards for their internal quality assurance processes, but may have their own industry standards to meet for accreditation purposes..
Examples include:
Aged care
Private Hospitals
If you were to look at these standards associated with these health care organisations you will note they still all contain reference and direct standards /criteria to person - centered care as part of their accreditation requirements, so it is still applicable to Standard 2:: Partnering with consumers
For this paper, you need to apply the resource identified to Standard 2:: Partnering with consumers’’ no matter what kind of organisation you are placed as it is the ‘ actions ’ from these standards to which you will be critiquing your chosen resource against..
What are the ‘actions’’??
The actions relate to each criteria from Standard 2:
Clinical governance and quality improvement systems to support partnering with consumers
Partnering with Clients in their own care
Health literacy
Partnering with consumers in organisation design and governance
The criteria in italics are those that are available for exploration to meet the requirements of this assessment (as detailed further below))..
Each criteria has a series of it ems and within those items are the related actions that provide cues for what the organisation has to demonstrate i n order to achieve the related item and therefore criteria under Standard 2.. There are multiple actions, but for this paper you must choose ONE from the following selected actions from Standard 2 to discuss .
Criteria:: Partnering with patients in their own care
Item::Health care Rights and informed consent
Action 2.3 The health service organisation uses a charter of rights that is :
a)) Consistent with the Australian Charter of Health care Rights
)) Easily accessible for patients,, carers,, families and consumers..
Action 2.4 The health service organisation ensures that its informed consent processes comply with legislation and best practice .
Action 2.5 The health service organisation has processes to identify :
a)) T he capacity of a patient to make decisions about their own care..
)) A substitute decision - maker if a patient does not have the capacity to make decisions for themselves .
Item:: Sharing decisions and planning care
Action 2.6 The health service organisati on has processes for clinicians to partner with patients and their substitute decision - maker to plan,, communicate,, set goals and make decisions about their cu
ent and future care
Action 2.7 The health service organisation supports the workforce to form
partnerships with patients and carers so that patients can be actively
involved in their own care
Criteria:: Health Literacy
Item:: Communication that supports effective partnerships
Action 2.8
The health service organisation uses communication mechanisms that
are tailored to the diversity of the consumers who use its services and,,
where relevant,, the d
iversity of the local community..
Action 2.9 Where information for patients,, carers,, families and consumers about health and health services is developed internally,, the organisation involves consumers in its development and review
Action 2.10 The health service organisation supports clinicians to communicate with patients,, carers,, families and consumers about health and health care so that::
a)) Information is provided in a way that meets the needs of patients,, carers,, families and consumers
)) Information provided is easy to understand and use
c)) The clin ical needs of patients are addressed while they are in the health service organisation
d)) Information needs for ongoing care are provided on discharge
The following
link
provide s a useful overview (aimed at consumers ) as to what they can expect from the care provided to them that is in alignment with Standard 2.. This may assist you identify resources (please look at ‘what can I expect’’ column )..
Additional information can be found via the following
link
. T his takes you to the accreditation resources link and within this , there are links to the Monitoring tools.. If you click on any of these,, it will open an excel spreadsheet.. If you then click on the ‘Partnering with Consumers’’ ta
sheet within this spread sheet , it will provide you with ‘evidence’’ of what constitutes that action being met.. Whilst it does go into a lot of detail as it is an accreditation tool,, this may help you to both identify and critique the quality of a patient centred care activity..
What are the resources I am looking for??
This assessment will require you to search for the resources with your PEP organisation. In some cases, you may be using them on a daily basis (ie . bedside handover templates ) or perhaps once a week (family meetings)).. You will need to observe practice in your PEP to see what activities appear to be person - centred and then to seek out the associated resources that inform
drive this process (eg.. policy and procedure,, professional development activities , patient literature,, etc) to be able to relate to the action you have chosen..
What is an example of a resource ?
Criteria:: Health Literacy
Item:: Communication that supports effective partnerships
Action:: 2.10
As an example , you may be undertaking PEP in a Community Clinic and cho o se to focus on those patients who are newly diagnosed with diabetes type 2 . A resource you may come across is patient literature around managing their disease and how it may impact on their lifestyle.. You would assume that this literature is written in a w ay that the patient understands and meets their needs. You could also assume that the patient has the opportunity to discuss with the health practitioner any questions they have and have direct in put into their care needs.. You could also assume that that literature was also originally developed in consultation with that particular health - consumer population. – This would demonstrate that this resource meets the action and is person centered in its approach. - So does it really ?
If this was your example, i n your paper, you need to consider ‘how’’ the resources have met the related action. Do you think it is person focused in its design?? How was this demonstrated
or not?? Did you notice different approaches by staff to educating the patient about what was contained in the document, did you think patients were welcomed
encouraged to contribute to their own care - planning?? Do you feel the patient left that interaction better informed?? If it was you as the RN involved in the implementation of that resource, what have you learnt from the experience and what impact would it have on how you conduct per son centered care in the future??
How should the paper be structured??
Choose the resource to review
It is important that you take the time to review your understanding of person - centered care to ensure you can recognise an appropriate activity and its associated resource to critique. You may need to discuss this resource with staff at your PEP organisation to determine how it was designed, implemented and evaluated to ensure you have adequate information for your critique .
Provide a
ief description of the resource and the context in which it was /used.. In this section, p ease also state the action you aim to critique this resource against..
Provide a
ief explanation of the importance of Partnering with consumers and its influence of patient outcomes in nursing..
Then, answer the following questions::
How does that resources meet the related action??
Here, you will need to relate how the resource meets the action (or not) you have chosen and support these findings with the relevant scholarly literature..
How effectively did you feel it is implemented in your organisation??
Here , you will need to explore how it is
was implemented.. Did you find staff were well educated on its use?? Did you find it was utilised differently by individual staff , not consistently,, or not at all?? On the contrary,, if you found
