Guidelines for writing your report
The report has three sections:
· Part A is an assessment of the client's psychiatric history and family/social environment;
· Part B is a mental state examination;
· Part C is evidence-based interventions.

Report presentation
The Mental health assessment and intervention report should be well planned.
For this assessment it is important for you to:
· Outline your introduction clearly and end with a concise conclusion.
· Make sure you justify your report by refe
ing to what was said during the interview.
· Write in complete sentences (i.e. do not use dot points).
· Address the points in Part A as a single section using complete sentences without using sub-headings.
· Use the points in Part B as sub-headings to structure Part B of your report.
· Make sure each sentence only deals with one issue. Keep sentences short, professional and to the point.
· In Part B the "formulation" section provides a summary of the information contained in the previous sub-sections. New information should not be placed in this section. If it is not in the body of a report it should not be in the formulation section.
· Does the client need to be refe
ed to a psychiatrist? Why?
· Pay attention to spelling, grammar and punctuation. It is important to proof read your work before submission.
· Writing style in a report is more formal than ve
al speech. Try not to write as you would speak.
· Use simple past tense to describe the patient's state at the time of the mental state examination. This emphasizes that the examination findings are specific to the time of conducting the interview.
After you have completed Parts A and B you can start Part C.
Part C requires you to research the mental health literature and recommend three evidence-based interventions to assist the client.
· Discuss each intervention separately and use a sub-heading so that the reade
marker know the intervention being discussed.
· What is the goal for the intervention?
· What is the clinical issue being addressed by each intervention?
· Ensure the three interventions you select are consistent with evidence-based treatment guidelines.
· Justify your decision by refe
ing back to evidence-based literature. 
· Make sure you appropriately reference your work using in-text references to support any statement of fact.
· Include a reference list on a separate page at the end of the report.

Introduction
The report assesses the mental health of a 21-year-old university student named ‘Andy’ who is being interviewed by a Psychiatrist for a Psychiatric Disorder. He is a student of final year of his degree and lives in a shared house with his friends. Andy is refe
ed to the Psychiatrist by his GP. The report presents analytical history of the patient, his mental state examination and the appropriate care plan that is best suited for the treatment of his problem.
PART-A: HISTORY
The client reported that his housemates work for Mi5 and they are putting stuff into his
ain probably against him. He said that he can hear them talking about him. He wondered that the housemates are always commenting about everything he is doing. For example, when he was packing the bag, the voices seemed to comment on that. He has the delusion of control as he reported that his housemates are inserting the stuff inside his
ain through Mi5 chip. The distu
ing thoughts are not his own. He has preoccupied belief that it is Mi5. He said that he can feel the chip inside his head over time. It’s a tracking device, rotating inside his
ain and he can feel its sensation. He is not sure about why his roommates are doing this to him. He did nothing wrong to them.
He did not leave his room much. It shows he had been isolated and depicting a negative behavior. The client also thought of taking overdose of some pills due to passive suicidal tendency. He used to smoke weed regularly. He has been taking ‘Speed’ drug and just last week he had taken it most recently. Other than that he is not taking any kind of medicine.
The client is uneasy, impatient and restless in his behavior. He is distractible and maintains a poor eye contact. Andy said he listened to the voices when his friends were not in his room and he was left alone. He also heard them at the time when he was talking to the Psychiatrist, just in another room. He seems healthy with rest of his body and cognition as well. He is not able to look after himself due to the distu
ance with the consistent voices. He is suspicious as he is fearful that his roommates might be putting some poison into his food.
The client seemed to exhibit Ideas of reference as he believed that his roommates are refe
ing to him or talking about him. He said that he feels safe at his parents’ house. And has a strong belief that his parents are not involved in this conspiracy. He does not want to drag his parents into all this and did not tell them about this. On being asked about whether talking to a psychiatrist or taking medical assistance would solve his problem, Andy said that He is not sure whether the psychiatrists would believe him.
PART-B: MENTAL STATE EXAMINATION
Appearance: The patient is confused, suspicious, socially awkward, isolated, Paranoid
Behavior: The patient exhibited a number of persecutory delusions, poorly understood behaviors and odd beliefs.
Affect: He takes long pauses in between the sentences; he does not trust anyone, hoards useless things.
Mood: Unhappy mood, suspicious, isolated, mutilates...
