Introduction to Assessment 2
Learner Instructions

For this project you will need to think of a Workplace Health and Safety Incident which happened in a workplace with you as an employee.
You are able to create the information you will use as the basis for this project or, if you work in a service or you have been on work placement, you are able to use information about that service.
If you use information about a real service, always use a different name so it cannot be identified. This is extremely important. Using a different name is called deidentifying the data.
This project will develop like a series of informal planning, evaluations and reflections, which would be worthwhile in the future when on work placement in or working in a service.
Please use each of the following numbered points as subheadings within your project and write your responses under each of these subheadings, and either write or, once complete, paste your project in the space provided. Please explain your project in detail (600 words).
1. What incident is the basis for this project? e.g., hurt back when lifting, dropped equipment on foot, etc.
2. What was the first response from the service to this incident?
3. What documentation was completed and to whom was it submitted according to safe work practices?
4. What consultations occu
ed as a follow-up to this incident?
5. Which sections of your State or Te
itory legislation is relevant to this incident?
6. Which sections of your service’s WHS policies and procedures are relevant to this incident?
7. What could have been improved to decrease the likelihood of this incident occu
ing?
8. How could the use of safety signs decrease the likelihood of occu
ence, if relevant?
9. How have the hazard identification procedures change in response to the incident?
10. How have the workplace emergency procedures changed in response to the incident? e.g., first aid.
11. What contribution did you and the injured person have in the development and implementation of changes?
12. How did stress and/or fatigue levels impact on the occu
ence of the incident?
13. How could have improved housekeeping practices reduce the likelihood of the incident?
