The focus of this review is to discuss how effective management and policy impact the healthcare organisations. Therefore we will have a detailed discussion on the evolution of the U.S health care system, the influence of the changing needs of the labour force due to increased technology, the aging of America, and various economic factors, the approaches of management and policy, will evaluate the impact of legal and regulatory policy on decision making and strategy development in healthcare organizations, will understand the risks or challenges a health care manager encounter and will also evaluate the efficacy of management practices in a contemporary healthcare organization.

The healthcare scenario in recent times is very different from what it used to be. The American healthcare system has undergone a huge change within two generation but also it is continuing to evolve very fast. It is changing due to the aging population, increase in healthcare cost, treatments getting costlier etc. In 1998 Bodenheimer and Grumbach defined managed care simply as "organizations that foot the bill for a patient's care have taken the role of managing the patient's care. Payers and insurers no longer simply write checks; they become involved in decisions about how much care a patient receives, what kind, and by which providers". Tradition and values in the 90’s had a great influence in the healthcare system which directly impacted the development policies and management policies. For example in the 90’s era most of the population were underinsured or uninsured as they did not paid much emphasis on having a full health insurance coverage. They used to seek medical assistance for any illness from emergency room, the most expensive form of health care services. 95 percent hospitals which provided acute care have emergency rooms units which used to remain open for 24 hours a day that are only designed to care for injured patients or acutely ill patients, and that the emergency rooms became so familiar that it became something of a family physicians for many individuals (Weiss & Lonnquist, 2000). Previously, during pre-industrial era i.e. from 1700s to 1800s medical practices was perceived as trade and not profession, primitive medical procedures were considered more effective rather than medicine, due to the lack of medical health care centres and institutions hospitals served as a social welfare and custodial function.
