In 750-1,000 words, develop an evaluation plan to be included in your final evidence-based practice project proposal. You will use the evaluation plan in the Topic 8 assignment, during which you will synthesize the various aspects of your project into a final paper detailing your evidence-based practice project proposal.

Provide the following criteria in the evaluation, making sure it is comprehensive and concise:

  1. Discuss the expected outcomes for your evidence-based practice project proposal.
  2. Review the various data collection tools associated with your selected research design and select one data collection tool that would be effective for your research design. Explain how this tool is valid, reliable, and applicable.
  3. Select a statistical test for your project and explain why it is best suited for the tool you choose.
  4. Describe what methods you will apply to your data collection tool and how the outcomes will be measured and evaluated based on the tool you selected.
  5. Propose strategies that will be taken if outcomes do not provide positive or expected results.
  6. Describe the plans to maintain, extend, revise, and discontinue a proposed solution after implementation.

Refer to the "Evidence-Based Practice Project Proposal – Assignment Overview" document for an overview of the evidence-based practice project proposal assignments.

You are required to cite a minimum of five peer-reviewed sources to complete this assignment. Sources must be published within the last 5 years and appropriate for the assignment criteria and nursing content.

Evidence Based Practice Proposal – Evaluation of Process
Evaluation is one of the critical steps in evidence based practice project (EBP) proposal implementation. It has various advantages such as
1. It allows EBP implementers to evaluate the effectiveness of the project proposal in achieving the desired outcome.
2. It enables EBP implementers to assess the effectiveness of the methods and tools employed in achieving the desired outcome.
3. It empowers the implementers to understand the project thoroughly and improve strategies for the project’s success.
Therefore, discussed here is the evaluation plan for the proposed project. In an attempt to decrease the falling rate and fracture incidences in stroke (cere
ovascular accident) patients, who are still under rehabilitation, it is important to evaluate therapy exercise programs to improve their balance. Providing physical therapy (PT) and occupational therapy (OT) exercise programs daily with range of motion every shift as compared to cu
ent practice of rehabilitation therapy 4- 5 times a week might be an effective treatment plan to reduce falling rates and hence fracture occu
ences in CVA patients during rehabilitation (Avanecean et al, 2017; Guirguis-Blake et al, 2018; Cheng et al, 2018; She
ington et al, 2017). Physical therapy is focused on improving or restoring movement, strength and range of motion, while occupational therapy is focused on improving motors skills necessary for daily tasks.
Expected Outcomes
The aim of this study is to create a evidence based intervention so that cu
ent practices can be improved and patients get effective results with that. Following outcomes are expected upon implementation of proposed intervention therapy.
1. Improved strength and range of motion in CVA patients due to proposed intervention.
2. Decrease in falling rate of cere
ovascular accident (CVA) patients under rehabilitation care over the specified period (3 – 6 months).
3. Improved nurse knowledge as they will be trained in patient centric fall prevention intervention like physical therapy and occupational therapy.
4. It is also expected that rehabilitation center will adopt the new intervention therapy instead of the standard care.
Data Collection Tools
The goal of the project is to compare falling rate of CVA patient in the intervention group (implementation of Physical Therapy and Occupational Therapy exercise Programs daily with Range of Motion Every shift) to the falling rate of CVA patients getting standard care. The effect size will be determined by comparing mean falling rate of both the...
