Patient was admitted to the facility approx. 2 months ago after numerous falls at home and acopia. Patient has been here long term whilst waiting for nursing home placement. Patient is diagnosed with early onset Dementia. Patient is being treated for Hypertension and Osteoarthritis.

Case History
During the shift, approx 11am, you are walking down the hallway and you notice that you can hear moaning and whimpering coming from a room. When you enter the room, you see another EN is in the room and is gra
ing the patients upper arms, trying to pull the patient forward from a recliner chair. The patient is crying, you can see tears coming down her cheeks and she is saying ‘No’ and is shaking her head. The EN states that the patient has soiled herself and needs to be changed but that ‘she won’t work with me, she’s being difficult’.
You know through handover, that this patient is usually continent and able to toilet independently. You know that she requires assistance with set up and is unable to get out of the chair due to her osteoarthritis. When you move forward to assist the patient with the EN, you notice red marks on the patients’ upper arms in the shapes of circles and scratches on the skin. Both arms seem to have these marks and the patient seems distressed when you approach her. You also notice that the bed is in the low low position close to the floor and that the patient would not have been able move in or out of the bed or chair, independently. You assist with changing the patient and adjust the bed position up to normal height and ask the patient if she would like to return to the chair or the bed.
A visitor enters the room and when the patient sees her, bursts into tears. The EN states ‘I didn’t know your daughter was coming you should have told us’. You assist the patient into bed and as you do this, the other EN turns to the visitor and states ‘we will leave you alone to catch up, she’s upset because she soiled herself’ and motions to you to go into the hallway. The EN states to you that ‘it was the patients fault because she wasn’t trying to get out of the chair early enough and that’s why she was incontinent’. You state to her that the patient wouldn’t have been able to get out of the chair by herself and should never have been left there. The patients’ daughter comes out of the room and asks ‘what happened? Why does mum have all these marks on her arms? they look like
uises and scratches? She is really upset and says that you hurt her’. The other EN states ‘I can’t deal with this ‘and walks away.

Scenario Information
Staff Member: Patricia Cleary 38 years. Enrolled Nurse has worked at the facility for 3 years as an EN but worked as a Carer for 4 years previous to that. She has stated that she loves the residents and they love her and has joked to you, that she doesn’t need to wo
y about her pension because they will look after her.
You are on placement at the facility, it is your 3rd week, for this afternoon shift you are allocated Patricia as your preceptor. You have been allocated with her on a number of shifts and have developed a good working relationship. It is 5pm and the residents are either in the dining room having their dinner, eating in their rooms or being fed in their rooms. On this shift, you are allocated to work in the blue wing of the Aged Care Facility with Patricia and have 32 residents. You have worked through a plan for the shift and you are preparing to do a residents dressing, as it got wet in the garden this afternoon. You have the dressing trolley ready and are transfe
ing it to the residents’ room to complete after they have finished their meal. As you enter the residents’ room you find Patricia in there and she is holding an antique clock.
She states to you “I’ve always liked this clock, my mum had one when I was a child and she gave it to my older

Patricia places back onto the shelf as the resident walks into the room with her 4WW. The resident states to Patricia “You really like that clock don’t you, it’s worth quite a lot now, shame I don’t have anyone to leave it to that likes it as much as you do”.
Patricia replies smiling at the resident ”Yes I do, but it’s worth more than money because it reminds me of you, I’ve told you, haven’t I, if you adopted me you would know exactly who you could leave it all to”.
The resident smiles at you and asks if you were going to do the dressing now and you reply yes. You complete the dressing with Patricia supervising, clean up and go to leave the room to get rid of your ru
ish. Patricia remains in the room talking to the resident, she states “We will have dinner once you have finished, I’ll meet you in the staff room”.

ish in the bin and have cleaned the trolley you go to the staffroom for dinner. You get your mobile phone from your locker and sit down and go onto Facebook/Twitter. You update your status in your class group (a private group for your classmates) stating you are on prac and are working with someone unprofessional this shift and do not know what to do …. you detail part of the incident when Patricia enters the room.
She is ca
ying the clock from the residents’ room and states “Isn’t that lovely, the resident gave me the clock, as she said she has no family and she wanted someone who would appreciate it to be able to enjoy it. I said no I couldn’t but she insisted”. Patricia places it in her locker and asks “What are you having for dinner it smells lovely”
