Introduction
Every day healthcare service providers have to deal with patients with diseases and sometimes patients with multiple health diseases. This assignment focuses towards one such case where Jim Cooper is dealing with multiple health issues and has been ordered to be transfe
ed to ward. Jim has some complex health issues, which needs to be prioritised over others. Further, the rationales for prioritisation will be provided along with the steps or measures required from the nurse regarding Jim’s care. Jim was entrusted to the registered nurse (RN), who was working in the morning shift because rush during morning is usually less and thus, he could be attended more thoroughly.
Patient’s Five Health Issues
Jim Cooper is being reported to have number of health issues and because has been ordered for transfer to ward because he lives alone and have no one provides him necessary care. From the report     Chronic Obstructive Pulmonary Disease (COPD), hypercholesterolemia, Gastro-oesophageal reflux disease (GORD), shortness of
eath (SOB) and fe
ile can be identified as the five major health issues. Other than these, high cough production, consolidation of lower lobe of left lung, high heart rate, tachy sinus and scattered noises upon auscultation are other symptoms that have been noted (Healthtalk, 2014).
Under the list of 3 highest priority health issues from the above selected five diseases, Chronic Obstructive Pulmonary Disease (COPD), heavy cough production, hypercholesterolemia will be kept and the rationales for the same is provided below. Other presented health issues have not been prioritized because of two major reasons. First most of other health issues are just side effects of these three issues such as fever and high heartbeat. Secondly, these three issues can be life threating in cu
ent scenario for Jim especially in short term while health issues can be treated with some extra time.

Jim’s chest X-ray report suggested that he is facing consolidation in lower lobe of left lung, which is the deposition of fluid or liquid, due to which air-exchange space decreases and less oxygen enters the blood. As mentioned by Bouhemad et al. (2015), this can be occu
ing majorly because of high cough production as evident from his reports and therefore, it can be considered as another reason behind shortness of
eath. Since, his lungs are filled with fluid therefore; air exchange is hindered and is another reason behind low SpO2 levels in his blood. Further, he is suffering from high fever and noises for his chest and these symptoms points towards the fact that he may be suffering from critical Pneumonia. Due to these reasons, high cough production should be considered as first priority.
Jim has been reported to have Chronic Obstructive Pulmonary Disease (COPD), which is a collective term to define a number of diseases such as non-reversible asthma, emphysema and chronic
onchitis. In other terms, Jim’s is facing destruction of air spaces in his lungs, which are responsible for the exchange of gases and this is the major reason behind his shortness of
