Mr.Kirkman, who is a 75 year old man, was admitted to Emergency Department for the complaints of Urinary Tract Infection and related symptoms. The patient has a history of previous UTI diagnosed by the symptoms like lower abdominal pain of scale 6/10, dark urine passed in small amounts in last 8 hours, presence of leukocytes, nitrite and blood in urinalysis. He has extremely high level of urea (15mmol/l) in the urine. High level of urea shows inefficient working of kidneys and urinary tract obstruction (University of Michigan, 2019). His blood count shows a high white cell count. His condition deteriorates after 24 hours. He exhibits the symptoms like low BP, mild fever and potentially high heart rate. It may be due to his severe sepsis and progression of infection.
The patient also had a history of Chronic Bronchitis. It may be due to his lifestyle that involves social drinking and heavy smoking, while increasing the risk of developing the lung infection. He uses his accessory muscles while
eathing. He has SpO2 82% showing obvious respiratory distress. Sever Sepsis involves infection with hypotension and is characterized by fever, pain, discolored skin, shortness of
eath, and unwell physical appearance (Sepsis Alliance, 2017). In this case, the source of infection might be the
onchitis. When bacteria enter the blood, the infection may spread causing leaking of blood vessels progressively worsening into organ dysfunction.
The untreated UTI may spread further to the kidneys, worsening the pain and ill symptoms leading to sepsis (Meddings, Saint & Krein, 2017). The sepsis occu
ing due to UTI is known as Urosepsis. It may occur due to any type of prevailing infection in the body. It is likely that Mr. Kirkman has got the UTI from his earlier infections of chronic
onchitis or a history of UTI.
The UTI may run from the kidneys, and passing through ureters and urinary bladder reaches the urethra. It may be caused by bacterial or fungal infection. During infection, the chemicals are released in blood resulting into inflammatory response. Such changes may initiate changes in body leading to failure and damage to multiple organs in the body.
