practice.

Community Practice Portfolio:
This assessment has four parts:
1. Introduce your Professional Practice setting and describe the health services this practice setting provides (200
words). Include artefacts (i.e. photos of the venue entrance, scanned
ochures, short videos, voice files etc.).
2. Discuss the roles and responsibilities of your preceptor nurse in the practice setting you have attended and
compare/contrast your observations to the roles and responsibilities described in the literature XXXXXXXXXXwords).
3. Find out how the nurses in the practice setting you have attended support, prepare and empower family
members/carers inorder to care for the patient/client (seek guidance from your preceptor nurse).

4. Critical Reflection on your community practice (600 words). This part must be structured under the following
subheadings:

*Identified areas for professional development;
*Impact on my future career.
Part 4 should be written in 1st person.
A critical reflection is a focused reflective activity, about professional situation/experience (negative or
positive) that had meaning and learning potential. Your reflection should incorporate some of the learned in
this course theories, for example, patient-centered care, therapeutic relationship or empowerment.
Assignment instructions: when creating artefacts, never take photos of staff, clients/patients (privacy), and avoid
tasking photos of the inside of the health facility without permission. Collections of artefacts must be created in My
ePortfolio, in VU Collaborate (step-by-
step instructions in VU Collaborate, in the Assessments module).
