The objective of this assessment is to assess your ability to effectively perform skills in the workplace to respond to the health requirements of two older people.

Instructions for the student
This assessment is based on activities you will undertake in the workplace. You will be permitted to use your reference text and learning notes when completing this assessment.
Your assessor will provide assessment results and feedback online, or via the feedback form at the end of this document.
Note: if you are submitting through an online platform, the assessor will give you feedback via the platform.
This assessment is in three parts:
1. Short answer questions for preparation before the provision of care for two older people

1b Short answer questions – Person with a dementia
2. Practical demonstrations – two direct observations by an RN of the provision of care for two older people:
2a Observation Checklist – Person with physical disability
2b Observation Checklist – Person with dementia

Submission instructions
Where possible, complete and submit this assessment online via TAFE NSW online learning platform.
Upload all the required assessment files in the assessment area on the online learning platform.
Alternatively, hand all required assessment files to your assessor for marking. Make sure you have added your name to the bottom of each page of the assessment.
It is important that you keep a copy of all electronic and hard copy assessments submitted to TAFE NSW and complete the assessment declaration when you have submit the assessment.
What do I need to do to achieve a satisfactory result?
To achieve a satisfactory result for this assessment all questions must be answered co
ectly and all parts of the observable components must be performed to a satisfactory level as indicated in the criteria section of the Observation Checklists.
If you do not achieve a satisfactory level, you will need to talk with the Course Coordinator or Head Teacher who may use their discretion to allow additional attempts.

Time allowed
Indicative time: allow a total of 6 hours to complete this assessment.
Part 1: 1.5 hours research and preparation (45 minutes per person)
Part 2: 4 hours in total
· 1 hour of discussion with registered nurse prior to implementing care (30 minutes per person)
· 1.5 hours for provision of care to an older person with a disability
· 1.5 hours for provision of care to an older person with dementia
Part 3: 30 minutes
Assessment feedback, review or appeals
Appeals will be addressed in accordance with TAFE NSW Policy. Refer to Every Student’s Guide to Assessment in TAFE NSW for more information.
Supervision
The written questions in Parts 1 and 3 can be completed at home without supervision.
The practical assessment in Part 2 must be supervised by a registered nurse. This will be either the clinical supervisor or a TAFE NSW assessor.
Specific task instructions
For this assessment you must demonstrate your ability to determine, perform and monitor appropriate nursing interventions that support the wellbeing of two older people, in consultation with a registered nurse. Your clinical supervisor will select the two people you are to care for, as some clients may not wish to have a student present or performing care. These could be the same people chosen for other workplace assessments completed on this placement.
To complete this assessment you must:
1. Analyse health care information and the clinical presentation of one older person with a disability and one older person with dementia to determine appropriate nursing interventions, then complete short answer questions in Parts 1a and 1
2. Discuss and confirm the appropriateness of the interventions you identified in Parts 1a and 1b with a registered nurse, then demonstrate the skills listed in the Observation Checklists in Parts 2a and 2b under the direct observation of an RN, including performing and monitoring the identified interventions. The RN may be your clinical supervisor or a TAFE NSW assessor.
3. Complete answers to reflective questions in Part 3.
Submit your completed assessment to your assessor in hard copy, by email or online by the end of your professional experience placement.
Instructions for your clinical supervisor RN are provided separately in the Professional Experience Placement Guide for Coordinators and Supervisors. Your supervisor will receive this from your TAFE NSW teache
assessor, who will liaise with them about your portfolio assessments.
Part 1: Short answer questions
Answer the following questions in the spaces provided based on your experiences while on professional experience placement, the person’s file, and conversations with the person, their family and/or carer as well as any further research undertaken, in consultation with a registered nurse.
Additional research/online research may be required to be undertaken during your own time outside of your professional experience placement. You must ensure you use reputable sources or evidence-based research to support your answers and cite the sources.
You will need to note the expected word length for each question.
In accordance with an appropriate role for a student on professional experience placement, you are expected to follow guidance from colleagues and a supervising registered nurse in regards to accessing any workplace documents or technology to complete this part of the assessment.
Part 1a: Short answer questions – Person with a disability
1. Review the person’s information (including case notes, care plan or any other information) and identify:
a) at least one of the age-related physical changes or transitions this person is experiencing
) at least one of the psychosocial needs of the person and their family or carer.
2. Based on the information in the person’s care plan, identify:
a) the age-related pathophysiological disorders the person is experiencing; and
) how they may affect the person and their family or carer.
3. Based on the information collected, including the person’s assessment data, progressive notes and medical history, identify two possible nursing interventions/actions that support the person’s needs. Your answer should be between 30 words and 50 words.
4. Provide a rationale for performing these nursing interventions. Your answer should be between 30 words and 50 words.
5. Outline how and when the interventions will be implemented, and the expected outcomes of the interventions. Your answer should be between 30 words and 50 words.
Part 1b: Short answer questions – Person with a dementia
1. Review the person’s information (including case notes, care plan and any other information) and identify the type of dementia.
