The objective of this assessment is to assess your knowledge required to contribute to the nursing care and management of a person with a mental health condition.

This is a project-based assessment and will be assessing you on your knowledge and performance of the unit.
Your assessor will provide assessment results and feedback online, or via the feedback form at the end of this document.
Note: if you are submitting through an online platform, the assessor will give you feedback via the platform.

Submission instructions
Where possible, complete and submit this assessment online via TAFE NSW online learning platform.

Alternatively, hand all required assessment files to your assessor for marking. Make sure you have added your name to the bottom of each page of the assessment.

To achieve a satisfactory result for this assessment all questions must be answered co

If your assessment is not satisfactory after two attempts, you will need to talk with the Course Coordinator or Head Teacher who may use their discretion to allow additional attempts.

Part 1: Case Studies with Short Answer Questions
Read each case study below and then answer the questions in the spaces provided. Ensure that you take note of the word count, where indicated.
Case Study 1: Jack
Jack is a 21 year old man who has been
ought into the emergency department by ambulance at 2am on Saturday night. He has some recent cuts on both wrists which suggest that Jack may have attempted to take his own life. Jack is highly distressed and is becoming increasingly agitated and just wants to go home.
You are working in the emergency department and have been asked to take Jack’s vital signs while the mental health team is contacted to undertake a mental health assessment.
1. Decribe four actions you could take as an enrolled nurse to de-escalate the situation with Jack and avoid the situation deteriorating further. Your response should be between 40 – 80 words in total.
2. Jack’s emotional presentation deteriorates further, and he is crying and restless. Briefly discuss who you would contact for assistance to ensure both yourself and Jack are supported. Your response should be between 20 – 40 words.

Case Study 2: Rita
Rita is a 48-year-old woman who lives in a group home with two other women. Rita has a diagnosis of paranoid schizophrenia and has been finding it increasingly more difficult to complete her ADLs and remember to take her medications. Recently she has been experiencing paranoid delusions that have been getting worse and more distressing for her. She hasn’t slept for the last few days, has gone off her prescribed medications and hasn’t been eating well.

ought into the hospital by a member of the community mental health team. She is admitted to the mental health ward where you are working today, and you have been assigned to provide nursing care for Rita during your shift.
You introduce yourself to Rita and find that she is very difficult to understand as she is speaking very quickly and is constantly changing the topic. Rita tells you that she is being followed by bad people who want to harm her and that she has voices telling her that she urgently needs to share her knowledge of these people with the Australian government so they can protect the community. She tells you she is very wo
ied that she won’t be able to help people now she is in hospital.
4. Outline four approaches you would use to communicate with Rita in a respectful and effective way. Your response should be between 40 – 80 words in total.
