Case Study
Kim, a Registered Nurse, graduated from the University of Technology Sydney in 2018. She
is now undertaking a Transition to Practice (TTP) program at a tertiary hospital in western
Sydney. Halfway through her program, she was involved in a cardiac a
est on the ward.
Fortunately, the patient was successfully resuscitated and transfe
ed to the Intensive Care
Unit. The resuscitation went smoothly apart from an incident where the attending Registrar
shouted at Kim, admonishing her for not knowing which equipment to provide for the
intubation. Up until that point, Kim had felt that she was “getting on top of things” and coping
quite well with the demands of ward work. Kim left the a
est feeling “hopeless” and “useless”
and incompetent as a nurse. These thoughts and feelings are causing her ongoing distress
and emba
assment, damaging her confidence and affecting her ability to conduct everyday
nursing tasks. For the first time in her career, she is considering leaving.
The following day, Kim’s Nursing Unit Manager (NUM) approaches her on the floor during the
usy morning shift (su
ounded by colleagues and patients). She says “I hear you were really
upset and might need some counselling after what happened during the a
est the other day.
I am free now, come into my office”. Kim follows the NUM into her office, feeling emba
assed
that the issue was raised in front of her colleagues and other patients. Kim also feels uneasy
about burdening her colleagues with her workload while she is in the office. Kim sits down
and begins to listen to the NUM….
3
Trigger questions you could ask yourself to put this paper together….
Issues and factors that impact on nursing leaders and professional practice
How would you approach the situation if you were the Nurse Unit Manager (NUM)? How
would the RN Standards of Practice apply in the NUM’s case?
Clinical leadership expectations for beginning new graduate nurses: scope of practice,
legal and ethical boundaries
Did Kim meet her obligations as a new graduate nurse? How would the RN Standards of
Practice apply in Kim’s case?
Gi
s model of reflection: strategies to develop personal and professional resilience
How you would have felt in Kim’s situation. What strategies would you recommend to
develop personal and professional resilience?
Supportive elements in effective leadership: knowledge, evidence, communication and
networks
What knowledge, evidence, communication and networks could leaders, such as the NUM in
the case study, offer new graduate nurses to support their development as a leader?

Introduction
Leadership is one of necessary aspects of life. Most of the individuals at some point of life have shown leadership skills or used their leadership traits to take the initiative or lead the people. In professional context, it has been seen that people take up the role of leadership while handling the team or being part of the team. In nursing practice, nurses often face situations where they need to act as a leader to handle patients, fellow staff members and family member of a patient. A nurse who failed to incorporate leadership skills in its practice often considered incapable. As mentioned by Northouse (2018), leadership can be defined as the way an individual used to lead people in a group, whether the group is a team in the organisation or people in the critical situation.
As mentioned by Lussier and Achua (2015), it must be noted that a person can able to lead people if they use co
ect leadership skills or leadership style. Development of leadership style, which suited the personality of the individual, come with experiences and practise. As per the case study, Kim is a new graduate nurse who joined the transition to practice program in the tertiary hospital. During the halfway of completion, the program, in her ward, she was involved in cardiac a
est case. Although, resuscitation process gone smoothly but registered nurse shouted at Kim as she was unaware of equipment, which provide intubation. This incidence left her hopeless and useless. She wanted to leave her practice as her confidence was damaged and she felt incompetent, and emba
assed which affect her ability to conduct.
Factors and Issues laying Impact on Nursing Leaders as well as Professional Practice
Nursing Unit Manager (NUM), called Kim to discuss the issue on the following day. The approaches used by manager worsened her feelings. As noted by Duff et al. (2015), the behaviour of manager towards the employees has direct impact on their working. Manger is considered as leader from whom it is expected a genuine empathetic behaviour. In case study, manager approach the Kim in front of everyone to discuss the incidence she was involved, which make her feel emba
assed.
The manager must have used the better communication mode to approach the Kim. I would have approached Kim either through email or by calling her on personal phone. In this way, she can reply me in isolation, instead of getting emba
assed in front of colleagues. As noted by Hartvigson et al. (2019), it has been seen that when person get emba
assed in front of their colleagues, this damage the confidence level of the individual. Kim was already feeling hopeless and incompetent, the raising of issue in front of her colleague while further worsen her feelings.
When nursing unit manager called the Kim, she was having guilt that her work is divided into her colleagues and their work pressure has been increased on rest of her team, which would leave them frustrated. As mentioned by Almost et al. (2016), it is the duty of manager to manage the team and make sure that in solving of the conflict, other people of the team may not be suffered. It has been seen that Manager called in the busy time to discuss the issue with the Kim, which hampered her work and burdened her colleagues, as a result Kim felt emba
assed. I would have called the Kim and asked about the time when she can talk to me. I would have given her assurance that I am available entire day, and whenever she is free and comfortable to talk, she can approach me. I would have asked her to visiting me is mandatory, so that she does understand the importance of the meeting.
When Kim went to meet the nursing unit manager, the manager started talking instead of asking about the feelings of Kim. Despite knowing that Kim is under distress, manager failed to address the issues. As stated by Gilley et al. (2015), it is duty of a manager to address the issues of the employees who are under distress. They must use the strategies to motivate the employees and reduce their distress. I would have allowed to Kim to talk about the incident instead of making her sit and listen the stuff. I would have addressed the emotions she is going through after the incidence. As suggested by Johnson and Smith (2018), it is a duty of manager to support the team member going through rough phase; they must help team member issues at work and provide them guidance for the improvement at work.
The manager must recommend Kim to counsellor, so that counsellor can give her behaviour therapy. This therapy allowed her to build her confidence back and change her feeling of guilt, and emba
assment. According to Nursing Standard...
