Introduction
In order to set the health priorities for future in hospitals or even at the community care centres health services plan is an essential aspect. Oral health in Western Sydney Local Health District (LHD) is a neglected health aspect. Therefore, numerous health plans that may address good health could be easily found however, a specific Health Service Plan (HSP) that targets oral health is not easy to find. This assignment focuses upon the development of a HSP that focuses specifically on oral health in Western Sydney Local Health District (WSLHD).
Environmental Scan
Policy and Planning Framework (External and Internal)
WSLHD HSP suggests that health services that are made available to the local or neighbourhood of individuals are mostly used. Further, this plan also suggests local hospitals as well as general practitioners as the centre of health care system. Therefore, externally this plan focuses upon moving towards a Patient Centred Medical Care Home (PCMH) model especially for Western Sydney. Further, internal policies in focus have been suggested as the development of excellent network that would reduce the waiting time as well as train the future or young generation. Special emphasis is provided upon children as well as aboriginal communities within hospitals.
Demographic Analysis
Under WSLHD HSP, services have been proposed and made accessible to every community or group however, certain groups have been identified as priority groups such as 68,980 children up to 4 years old have been prioritised. Along with this, 68,210 adults above 70 years of age, 13,390 aboriginals, 120,080 individuals between 15 and 24 years of age and individuals with chronic or mental diseases have been prioritised.
Relevant Population Projections
According to the data provided by HealthStats NSW (2019), the expected population of Sydney LHD is around 385,000 by 2026. Further, they have suggested that the increase in population is not drastic and is expected to be uniform every year. Likewise, the ratio of males to female is expected to be 50-50 in almost every age group. It is essential to consider that WSLHD HSP is designed until 2026 and these population trends have been given specific emphasis moreover, with the population projection the plan appears to be strategic as well as in accordance with the uniform population increase (HealthStats NSW, 2019).
Determinants of Health
In order to address the increasing needs for healthcare services in coming future access to healthcare and health services have been given specific emphasis. It is evident from the goals of this health plan where providing healthcare services near home or in neighbourhood as well as care at home has been focused. Further, another major determinant of health is quality health care and social support. Concept of integrated health services and the goal of connecting and increasing networks reflect upon the social support as well as improvising healthcare services.
