The process of transitional health care is mostly considered as a long term process of care. This transitional care policy generally deals with the process of intense care of the patients’ wound as well as therapeutic care and medical support of the chronic disease (Coffey, et al., 2017). In the vast discussion of the given scenario, it can be observed that the transition health care systems are facing a series of problems and various issues, and those are needed to be resolved.
