Task context and aim:

Community health nurses are required to have a good
knowledge and understanding of theoretical concepts relevant to
community nursing practice. This includes being able to analyse
elevant data and identify social determinants of health and
engage with vulnerable groups. Detailed needs assessment is
important to inform health promotion planning and practice.
The aim of this assignment is to demonstrate your knowledge and
understanding of important community health nursing concepts and
principles, including: social determinants of health, identifying
and working with vulnerable groups, principles of community
engagement, undertaking detailed needs assessment and
health promotion planning.
Specifically, you will undertake analysis of a real community
data set, identify and discuss relevant social determinants of
health, identify a vulnerable sub-group, explain how to engage
with the community and how to undertake a more detailed needs
assessment. Finally, you will explain how this information from a
needs assessment could be used to inform health promotion
planning in this community.





Task description:

There are two parts to this assessment item, Part A and Part B.
Select one (1) community data set ONLY, either Mt Isa Or Inala
Part A: Using one of the ABS, 2016 data sets provided in Appendix
B, demonstrate your understanding and analysis of the data by
selecting three categories of data relevant to health, and developing
three (3) comparative summary statements and three (3) inference
statements. You will populate this information in Table 1 in your
assignment (maximum of 150 words).
Part B: You are to write a 1,350 word essay in which you will discuss
social determinants of health in relation to identify a vulnerable sub -
group of people and discuss the process of community engagement,
needs assessment and health promotion planning.
What you need to do:

Part A: (150 words)

Create and populate a table (Use the Appendix A: Assignment
Table 1 guide provided below):

a) Choose one of the communities in Appendix B – either Mt
Isa OR Inala.
) Carefully analyse the Appendix B: Community Data for
Analysis table for your chosen community (INALA) and
select three (3) categories of data that are relevant and
linked, social determinants of health

that compares the local data to state and national
average data, including percentages where relevant

    The median age of Inala community is 33, which is lower in comparison to the state median age, which is 37, and countries median age, which are 38.
    As suggested by Niba et al. (2019), median age is likely to determine the condition of health in a community. In cases of Inala community, as median age is lower than the median age of state and country, it means community has higher number of fertile and younger generation.
    Education
    12.7% of people are studying in degree courses in comparison to state and country, which is 18.3% and 22% respectively. 5.4% of population is enrolled in diploma courses, which is lower than the state (8.7%) and country (8.9%) statistic. The percentage of lower education such as certificate course, year 10 and year 11 is higher in community in comparison to the state and country.
    As suggested by Rosch (2018), the ratio of higher education is drastically low in the community, which prevent them in taking high-skilled jobs. Low education rate is usually linked with low health literacy rate. This can influence the health of the person directly.
    Industry of employment
    18.6% population is engaged in copper mining in comparison to state and country, which is 0.1%. 9% population is engaged in silver-lead and zinc mining in comparison to state and country, which is 0.1 and 0 respectively. 4.6% of population is engaged in hospitals and 3.9% in education, which is comparatively higher from state and country ratio.
    As suggested by Rhee et al. (2018), there is direct impact of type of employment on health. For example, when a person works in busy hospitals, they likely to develop stress on the body

Introduction
    Inala community is a subu
an region is city of Brisbane, Queensland, Australia. The population of the community is around 61,000. The people of different origin such as Australia, Vietnam, Samoa, India and New Zealand live here. Around 4.5% of the population is the indigenous population. People of the community are mainly dependent on mining for employment. The higher education percentage is quite low in the community but basic education is taken by most of the population. As stated by Garg, Toy, Tripodis, Silverstein and Freeman (2015), there are many factors, which affect the health of the community, which also make the people of the community vulnerable. This essay is a community assessment that deals with analysing the social determinant of health in community and how they are related to each other. It identifies the vulnerable group based on the developed data of Australian Bureau of Statistic 2016 and assesses the need of vulnerable group and plan the health promotion activity to support the group.
Analysis of social determinant of health
    There are many factors, which are responsible for the determinant of the health of the community. As suggested by Garg, Toy, Tripodis, Silverstein and Freeman (2015), these factors are called social determinant of health. Some of the social determinants of health are age, literacy, higher education, employment, housing, income, domestic violence and substance abuse.
