Introduction to the Presented Case (S: Situation)
Mr. Luis a 65-year-old man diagnosed with idiopathic pulmonary fi
osis and COPD.
Initially, Mr. Luis was diagnosed with nifedipine; however, condition worsened.
Multiple inhaled medications and inhaled corticosteroid were proposed.
The presented case study for this paper includes 65-year-old man, with the history of smoking. Mr. Luis has been reported to suffer from worsening exertional dyspnea. Further, pulmonary function testing reflected a significant restrictive lung disease, which later was infe
ed to be secondary condition to patient’s already existing COPD. Likewise, a mean pulmonary artery pressure (PAP) of 52 mm Hg was observed from two-dimensional echocardiography; however, the functioning of right ventricle was found to be normal concerning size and functioning. Moreover, at the time when diagnosis was confirmed, right heart catheterization confirmed elevated PAP with 12 mm Hg as the capillary wedge pressure along with peripheral vascular resistance of 7 wood units.
Mr. Luis was diagnosed with concomitant idiopathic pulmonary fi
osis and COPD. Initially, Nifedipine was found to be effective for Mr. Luis and he was asked to visit again after 2 months; however, this time Mr. Luis returned with the issue of shortness of
eath even due to minimal activity.
