· Age-65 years
· Biologic Gender- Male
· Ethnicity- Welsh
· Occupation- Professor of Economics at National University of Economics for past 25 years
· Social History- Lives with wife and two sons
· Smoking Status and ETOH use- Active smoker for past 30 years and consumes ETOH only occasionally.
· Medication list- Glycomet-GP 0.5 forte tablets, Zapiz-0.5 as hypnotic, Montaire-10 for allergy, all of which are taken regularly.
· Medical History- Dia
hea at about 10 years of age; Hepatitis A infection at the age of 20 years; Diabetes Mellitus for the past 15 years; Asthma for the past 10 years.
· Surgical History- Appendix operation in early 30s and Cataract surgery at 57 years of age.
