Introduction
The health of individuals, as well as communities, is dependent on the social, economic as well as environmental factors. As per the World Health Organization, the health of environment and circumstances affect the wellness and health outcomes (The determinants of health, 2019). Access to healthcare is also one of the important factors that determine the health and wellness of an individual or a community. In this report, the social determinants of health-related to the ageing and care services with respect to the provided case study will be assessed. The discussion will also include the relevant policies that help the person-centred care and active ageing care activities.
Social Determinants of Health
The living conditions, genetic construct, education levels and income along with the social relationships within the residence will affect the state of the health for individuals and community members. It is essential to consider the context of the lives of individuals in regards to poor health. The income and social status of the individuals ensure that the gap of healthcare was minimised and led to lowered differences in health (The determinants of health, 2019). The lowered education level of the individuals would be susceptible to poor health outcomes. The stress, lack of confidence and awareness are some of the reasons for which the individuals or community members did not receive access to good health. The physical environment of the individual or community also determines the health outcome (Artiga & Hinton, 2018). The impact of the safe residence, workplaces, clean air and water influences the health and wellness of the individuals or community. Good workplace environment and cultivates employments within a particular community. The better opportunities for employment would increase the income as well as the healthier lifestyles. The infrastructures also impact the physical environment within the community like roads would provide better access to health in an emergency (Artiga & Hinton, 2018). A social determinant also includes the support for the social group of the individuals. Better health is achieved with the support and involvement of the community members, friends and the families of the individuals (Purnell et al., 2016). In this regard, the impact of culture plays an important to sustain better health within the community. The beliefs, traditions and customs that are shared between the social group

The propensity of individuals to develop certain illnesses, lifespan and the health outcomes are impacted by the genetic construct. The influence of the eating, drinking, smoking and coping skills also affect the genes, which can be altered to become vulnerable to diseases. The utilisation of the health services and access to healthcare can facilitate the health outcome of the individual. The gender difference may or may not influence the susceptibility to diseases during different times in their lives.
Issues with Aging and Care
The geriatric population consists of a significant proposition of the Australian population. It was estimated by the Australian government in 2017 that 1 in every seven people was over the age of 65 years (Australian Institute of Health and Welfare, 2016). This reflects the underlying health issues that need to be addressed with the growing geriatric population within Australia and its associated disease burden. In 2016, the estimations revealed the majority of the geriatric population living in Australia were originally from a different non-English speaking country. This poses difficulty in the healthcare workforce to provide services and communication. The population census showed that men over the cu
ent age of 65 would live for another 20 years and women in the same age range would live for 22 more years (Australian Institute of Health and Welfare, 2016). The increased burden of disease would be the impact of the socio-economic condition of the country after the premature life loss. This disease burden can have a non-fatal impact on the society with the individuals suffering from ill-health impacts and live years with disability, the cost of associated health services expenses need to be provided by the individuals as well as the government (O’Doherty et al., 2016). The direct impact of the ageing population would be observed in the changes in the health system in regards to the health profiles and increased demand for care within the industry.
