Case Study
Kim, a Registered Nurse, graduated from the University of Technology Sydney in 2018. She
is now undertaking a Transition to Practice (TTP) program at a tertiary hospital in western
Sydney. Halfway through her program, she was involved in a cardiac a
est on the ward.
Fortunately, the patient was successfully resuscitated and transfe
ed to the Intensive Care
Unit. The resuscitation went smoothly apart from an incident where the attending Registrar
shouted at Kim, admonishing her for not knowing which equipment to provide for the
intubation. Up until that point, Kim had felt that she was “getting on top of things” and coping
quite well with the demands of ward work. Kim left the a
est feeling “hopeless” and “useless”
and incompetent as a nurse. These thoughts and feelings are causing her ongoing distress
and emba
assment, damaging her confidence and affecting her ability to conduct everyday
nursing tasks. For the first time in her career, she is considering leaving.
The following day, Kim’s Nursing Unit Manager (NUM) approaches her on the floor during the
usy morning shift (su
ounded by colleagues and patients). She says “I hear you were really
upset and might need some counselling after what happened during the a
est the other day.
I am free now, come into my office”. Kim follows the NUM into her office, feeling emba
assed
that the issue was raised in front of her colleagues and other patients. Kim also feels uneasy
about burdening her colleagues with her workload while she is in the office. Kim sits down
and begins to listen to the NUM….
3
Trigger questions you could ask yourself to put this paper together….
Issues and factors that impact on nursing leaders and professional practice
How would you approach the situation if you were the Nurse Unit Manager (NUM)? How
would the RN Standards of Practice apply in the NUM’s case?
Clinical leadership expectations for beginning new graduate nurses: scope of practice,
legal and ethical boundaries
Did Kim meet her obligations as a new graduate nurse? How would the RN Standards of
Practice apply in Kim’s case?
Gi
s model of reflection: strategies to develop personal and professional resilience
How you would have felt in Kim’s situation. What strategies would you recommend to
develop personal and professional resilience?
Supportive elements in effective leadership: knowledge, evidence, communication and
networks
What knowledge, evidence, communication and networks could leaders, such as the NUM in
the case study, offer new graduate nurses to support their development as a leader?

Introduction
Leadership is a complex yet an essential and mandatory concept inn every industry. Leaders are required to guide their team or other members working under them to achieve the goals as well as objectives of their respective organisation. At the same time, leaders are also required to help their team members with the issues that they may face. Effective leadership in healthcare is required in order to deliver high-quality care and ensure patient’s safety along with fostering positive staff development. Nursing leaders are required to reflect creative an innovative personality so as to support the new nurses. Nursing profession and the associated leaders would be required to train the newly appointed nurses on operating latest technology and other complex medical equipments. Instead, as evident from the case study these nurses may not even receive the opportunity to imply basic leadership principles. This assessment focuses upon Kim’s situation and efforts will be made in order to explain how implication of knowledge, evidence, communication and networks effective leadership could have changed the situation.
Issues and factors that impact on nursing leaders and professional practice.
Nursing Unit Manager (NUM) are registered nurses that possess the educational qualifications as well as have 3-5 years of nursing experience allowing them to effectively supervise nursing units in hospitals and other healthcare organisation. NUM are required to maintain as well as sustain qualified staff members that possess the ability to operate unit in association with assuring patient safety and other routine operations of medical unit. Adams, Chamberlain and Giles (2019) suggested that NUM also possess majority of leadership and administrative responsibilities in accordance with facets of nursing.
Further, it would be necessary to consider that NUM are responsible for managing the cases as well as nurse’s engagement with respect to a specific case. It would also be essential to consider that in the presented case study, Kim is has recently graduated from nursing school. Therefore, she may have not been able to receive much experience related to dealing with severe cases such as that of cardiac a
ests. This incident where Kim was shouted upon by the registrar have been reported to affect the motivation, confidence, hope as well as ability to conduct everyday nursing task.
According to me, cu
ent response from NUM where she points out the incident in front of other patient’s and nurses is inappropriate as well as unprofessional. This is majorly because as suggested by RN standard 5, 5.1- RN are required to collaboratively construct nursing plan until options, goals and priorities are being agreed with relevant persons. Therefore, NUM was required to follow this standard, instead of calling Kim in front of other nurses and patients I would have talked to Kim by taking her to some corner or other specific area where external distu
ance was not there. Manning et al., (2015) mentioned that NUM or Nurse in charges are not allowed to discuss anything related to other nurses or practise nurses with anyone. Further, their major role can be mentoring of new nurses or practise nurses.
Confronting Kim privately and not in front of other nurses would have been my strategy to deal with the situation. Kim was already having low confidence and was doubting her abilities after getting scolding from registrar and further reprimanding her in front of patients and other nurses could have humiliated her.
Moreover, RN standards 6.2 and 6.3 suggest that RN are required to deal with other nurses within the scope of their practice which is to confront them or discuss sensitive situations privately. Likewise, I would have asked her to meet me in the office during lunch time to discuss her practise so that her work was not effected and moreover, she would have not felt humiliated.
