For this assignment you must effectively demonstrate your understanding and skills in managing quality in Health and Social Care Organisations. To do so, you must carry out the tasks below in relation to the CQC report for Rose Meadow (RMCH). Carefully read the CQC report for Rose Meadow (RMCH). The CQC report showed a need for urgent improvement of quality in the care home. As a Quality Manager you are responsible for managing quality. You are required to identify and make required changes to improve and manage the recommended quality before the next CQC inspection. To carry out this task, you must carry out research on relevant available literature/data sources. It is advisable to consult relevant sources of literature and data (e.g. free NHS or CQC online reports and texts) on the organization to fully understand the context of managing quality.

Description of the problem to be solved

The CQC report identified some improvements and a need to further improve how the care home assessed and monitored its quality and safety procedures. As a health and social care practitioner (Quality Manager) you are required to provide a report explaining the systems, policies and actions that can be taken to tackle the identified shortfalls.

Assignment

Based on the Case Study above you are required to provide a Report of approximately 4,000 (+/- 10%) words. (The word count is set here as a guide.) You should refer to the assessment criteria and the relevant unit content, when preparing your evidence of assessment. The report should address the following four tasks below which relate to the learning outcomes.

Section 1

In section1 of your report you are required to show an Explanation of perspectives that stakeholders in health and social care have regarding quality; Analysis of the roles of external agencies in setting standards and Assessment of the impact of poor service quality on health and social care stakeholders. You need to refer to the case study provided to cover all the assessment criteria.

Section 2

In section 2 of your report, you need to show that you understand strategies for achieving quality in health and social care services. This will be achieved by analysing potential barriers to delivery of quality service; evaluating different approaches to implementing quality systems and finally explaining the standards that exist for measuring quality at Rose Meadow.

Section 3

In section 3 of your report, you are required to evaluate systems, policies and procedures in Rose Meadow. You are required to evaluate the effectiveness of systems, policies and procedures used in RHCH in achieving quality in the service(s) offered. You should also analyse other factors that influence the achievement of quality, and lastly suggest ways in which quality could be improved.

NB: You must identify the specific systems, policies and procedures that you are evaluating in this section. This can include any systems, policies and procedures mentioned in the CQC REPORT.


Section 4

In section 4 of your report, you are required to evaluate methods for evaluating quality with regards to external and internal perspectives and discuss the impact that involving users of services in the evaluation process has on service quality.

The feedback is for your benefit and is not part of your final grade for the unit. Formative feedback may, however, not be as detailed as the final feedback you receive, and may be in verbal and/or written form (at the lecturer’s discretion). You will be asked to submit your work for formative feedback in electronic or hard copy form to which you will receive verbal and/or written feedback.

Final (Summative) Submission

Grading

Each Unit will be graded as a Pass, Merit, and Distinction when all learning outcomes and assessment criteria have been met.

A Pass is awarded for the achievement of all Learning Outcomes against the specified Assessment Criteria.

A Merit or Distinction is awarded for higher level achievement upon achieving the M’s and D’s criteria.

If all required assessment criteria have not been achieved the work is graded ‘Referred’.

If the work illustrate more than the similarity % cap or concern over originality of part or all content of the work, it is graded ‘Withheld’. The student will then be interviewed to ascertain originality.

In the case of incomplete work or partial submission (others such as blank template, no evidence of real attempt), the work is graded ‘Unreasonable Submission’.

Quality can be defined as the standard of measuring on thing while comparing it with other. For example, nurse in the care setting providing patient-centred care is a quality service.
Managing quality in social and healthcare was one of the main priorities of healthcare system. Central of all the healthcare services is customers. All the healthcare organisations tirelessly working to find the ways in improving the quality of their services so that the care provided to the customers are the best. Quality depends on the perspective of the person.
Quality in healthcare can be measured on many factors such as safety, efficiency, time and effectiveness. The healthcare organisations, which are successful in managing quality of their services, can develop the standard of their work. This standard helps in building the reputation, which in return increases the business of the organisation as people willing to take services from those care settings, which have good reputation in the market.

Stakeholders can be defined as the people who are involved in business. As mentioned by Kilbourne et al. (2018), in social and healthcare system, some of the key stakeholders are patient, service providers like doctors, nurses, physician, social workers and staff members like manager, and staff, pharmaceutical, owner of healthcare system and inspecting bodies. Patients are one of the key stakeholders who seeks the quality care. For example, elderly people seeking refuge in nursing homes. As noted by Esmail et al. (2015), elderly population living in the care settings find it difficult to be adjusted. Elderly people shifted to the care setting to get the help in completing the daily routine. Elderly people expected that they are taken care properly by nurses of the setting. They must be provided medicine on time. They were given assistance in completing their daily routine. They expected to not to feel alone during their end life.
Another stakeholder is nurses in the healthcare system. Nurses play the wide range of responsibilities at their work. They consider helping patients as part of their job. It is a social duty which nurses are performing by providing quality care. Nurses expected kind gesture in return for providing quality care to the patients. For example, they expected appreciation for their work or considering their hard work. The duty of nursing staff is to provide safety to the patient. They helped in completing the daily routine of the patient.
Manager are one of the important stakeholders who manages the healthcare setting. They sort out the issues of conflict between nurses and doctors, and issues generated by patients. Duty of manager is to take care of regular maintenance of the supply and manage the facility. The legal bodies set the benchmark for the quality of the services to ensure that patient achieve the quality. They make sure that the benchmark set is achieved.
Every stakeholder judges the quality of the care setting based on their perspective and desired outcomes. As mentioned by Gradinger et al. (2015), workers or staff members work with the perspective of getting incentive. Staff members provide high quality service to the patients if they are rewarded or recognised for their work. As per the case study, there is absence of registered manager in the Rose Meadow Care Home (RMCH). There was poor management in the care home. Lack of availability of the staff members at co
ect position makes it difficult to meet the need of every member of the care setting. Staff members are failed to follow the guidance plan laid to every member of the facility. The health condition of the people is vulnerable to diseases. The care setting is unclean and there is risk of diseases. The care setting has poor quality and high risk of fire. The maintenance of the care setting is poor. Manager tries to assert that they are doing in the best capacity.
1.2    Analyse the role of external agencies in setting standards
A. Briefly explain your understanding of external agencies in HSC?
External agencies play the important role in making sure that quality of healthcare setting is up to the benchmark. They regularly audit the healthcare settings, inspect them and formed the report on it. Their report consists of loopholes in care, level to quality benchmark is achieved or not, and they also provide suggestions for the improvement. They play important role in closing of the healthcare settings.
B. Analyse the role [what they do, how they do] of any three external agencies related to case study with the importance of their standards setting (analyse each in separate paragraph).
There are two major external agencies in health and social care, which set the standard of the quality care given to the patient. These agencies are care quality commission (CQC) and national institute for clinical excellence (NICE). As suggested by Boyd et al. (2018), health and social care of 2008 act led to the establishment of CQC, which helps in regulating the health and adult social care services in England. The CQC inspector measures the institute against the standard. The care provided by local bodies, national health services, private companies and another voluntary organisation is regulated by CQC. The CQC aims to make sure that the care settings provide the high-quality care services to the patient.
As per the case study, Rose Meadow registered themselves in 2011 for the inspection. Since then inspection occurs every year. The recent inspection was conducted in 2018, which was surprise inspection. During the inspection, inspector checks the quality of care, cleanliness of place, management, safety of patient and storage of medicines. CQC can take the appropriate action against the care settings whose standard is lower than the set standard. CQC report provides guidance to the care setting to take care of necessary loopholes in their care. This report assesses the care settings very properly, which help care settings to identify the risks, which can happen. For example, In Rose Meadow, CQC inspector found out that the place is at risk of getting fire easily.
Another important agency is National Institute for clinical experience. This is an independent organisation, which helps in directing the ways, in which care settings can promote good health in their care settings. They provide guidance in prevention and treatment of diseases (Van Zwanenberg and Edwards, 2018). For example, in Rose Meadow, they helped in providing the care plan for all the patients so that the care provided to the patient fulfil their needs and its quality is very high. Service providers and professionals are given training to handle the patient medication and care under NICE.
Another important agency is MHRA which is medicine and health product regulatory agency. It is a governmental agency which ensures the quality of medicines and medical devices. It audits the safety level of the medicines and medical devices. It is an executive agency of the department of health. This agency is responsible for the regulation of medicines and medical devices. It investigates the harmful incidences. They analyse blood work, healthcare providers who provide blood and work for other organisation which can help in improving blood services.

A. Briefly explain your understanding of poor service quality
Poor quality services are the services provided to the patient or service seeker by healthcare service provide which failed to meet the benchmark set for the good or high quality of the services. Poor quality services can reduce the quality of life of the patients.

Patients or service seeker are the main stakeholders who get affected by poor quality service on health. As suggested by Nambiar et al. (2017), poor quality care when given to the patient or service seeker, they existing health start deteriorating even further. It has been noticed that people can become vulnerable to many other diseases. Poor quality care involves unhygienic environment, medication e
or, failed to manage the care plan given to the patient, use of poor equipment and poor storage of medicine. Poor quality can be fatal for the service user. They can get exposed harmful microorganism, which can cause many kinds of infections in service user. The poor storage of medicine results into drug allergy, which is very dangerous. As per the care study, Rose Meadow has poor storage of medicine; there is lack of safety of the patient. Patients are vulnerable to the infectious disease due to lack of cleanliness in the place. Poor quality does not keep the organisation untouched.
Another stakeholder which get affected by the service is owner or manager of healthcare organisation. It ruins the reputation of the place, which led to the loss of the business as people avoid all those healthcare organisations that failed to provide the high-quality care to the patients. CQC and other regulatory agency have power to take necessary actions against the organisation whose quality is not up to the mark. As mentioned by Salyers et al. (2017), it has been seen that organisations can be shut down by authorities if care quality is low. Nurses and other staff members who failed to provide the quality care can face the legal suit against them. They can loss their job and their reputation will be ruined, which can lead to the unemployment in their field. As per the case study, CQC report of Rose Meadow shows that organisation failed to provide high quality. They have directed few instructions to the organisation so that quality can be improved otherwise necessary action will be taken against the institute.
