Introduction
Care plan is an imperative part, which provides the ultimate care of an individual and a proper direction for an individualised care. Care plans should be patient centric, in other words, which implies that it should be customised according to the specific needs of every patient. Hence, no patients should be considered in general and even if their situations or illnesses are common, their problems must not be generalised. Hence, care plans are designed in such a way that they meet the continuity of the care needed for a patient where it helps by communicating with the patient and organising a perfect life schedule and plan, which will ultimately help in their wellbeing. In the provided case study, Mrs X, 72-year-old widow, is suffering from a massive stroke. As she is widow, and her family living far away, therefore, she is all alone in her property. She had to work the entire household though it was becoming quite stressful. In this essay, the system and model structure for the assistance of Mrs X has been described and with the help of the present caring skills, the assessment of Mrs X has been performed. The managerial view on the outcome of Mrs X as well as her customised care plan and a proper advice has also been delivered to the care provider of Mrs X.
Q1. Systems, Structures and Models Relevant for Assessment of Mrs X’s Support Needs
Mrs X was being able to cook and take care of her household. Besides, she goes for lunch outside and meets her friends using a public transport. However, as per the report of the care provider, since she really needs an assistance for her household chores, it would be more liable if the food and grocery can be supplied online to her house because she also cannot drive to the stores for shopping. Moreover, as she feels difficulty bathing due to there being no fitted shower in her bathroom, therefore, it has also been suggested that the care provider could help her in and out of her bathroom, when she has to take a bath.
It is to be noted that all these needs assessed about her are not mere expression of the lot of mobility and loneliness in her. It is rather a lot deeper issue that can be analysed with the help of biopsychosocial, social, medical, empowerment and holistic models. Firstly, by implementing the biopsychosocial model, it could be seen that Mrs X is lost her husband and she feels lonely. As mentioned by Towfighi et al. (2017), the biopsychosocial model of health and illness states that a person can become ill due to biological, social and psychological factors. In other words, it can be stated that it provides a framework, through which it can be identified that the occu
ence, clinical manifestation and results of an illness can take place only when the above-mentioned three types of factors interplay. Therefore, in case of Mrs X, the care provider can assess the way to make the person happy by communicating directly with that person.
On the other hand, using the social model of health determines the occu
ence of illness or bodily distu
ances in a person in a very different way than the biopsychosocial model. According to Smith (2017), the social model takes up all the existing factors in to consideration, when it comes to analysing the actual reason behind a certain illness. Hence, according to the social model, apart from the physical issues, the environment, in which the person lives, their society, culture and political aspects, also contribute towards their illness. Thus, social loneliness hampers in wellbeing and the conversation may always not be that much easy for the care provider. They cannot figure out what kind of help will be actually good for the person. By offering her household tasks, managing time for them, sharing meal, seeking any kind of discomfort or winter sickness, helping them by providing useful resources may help the patient to recover from the loneliness.
The holistic model states that when a person is ill or lonely, they need to be cared for from all dimensions that are their care procedure should be covering all of their aspects, whether it is mentally or physically. This is refe
ed to as the holistic approach of care, which also turns into a person-centred care. Hence, in case of Mrs X, this model can also support assessment of her needs by empathising with her. As per the view of Schreuders et al. (2017), treating of such a person who has mobility discomfort and feeling lonely, it becomes imperative when one have developed such a depression stage and the symptoms are started to appear. The untreated period lasts for around six months to several years and hence it is most important to solve the problem as soon as possible. A skilled therapy may help the person who is giving the care and this will help to manage the life of such a person in a better way.
Furthermore, using the empowerment model, another aspect of Mrs X’s illness can be supported to be assessed that is depression. It can be seen that a lonely person such as she, living and working in her house, all by her own, is suddenly left to feel that independently, she cannot perform her household chores. This can lead to depression from her loneliness and mobility issues, which can be assessed by communication and if the person can find a person, to whom she can share her problem. As mentioned by Keyes et al. (2015), disability is quite challenging that develops the state of depression.
Lastly, as the medical model states, every disease can be assessed, diagnosed and treated like a physical issue, i
espective of the fact that it can be a psychological or mental problem (Oliver, 2017). Hence, in case of Mrs X, since she has mobility issue, her needs could be assessed using physical tasks, the performance of which could determine its extent of intensity, while her loneliness can be related to depression, which can be physically assessed as a consequence of not having close ones with her.
Apart from these models, there are also certain frameworks that thoroughly support the assessment of support needs for such patients as Mrs X, such as ASPIRE framework and Warwick-Edinburgh Mental Well Being Scale. As stated by Ghitza et al. (2015), the ASPIRE framework is an online learning support for the care providers, as well as the patients themselves, in order to identify their support and health needs. It is a learning tool, with a number of technological aids such as Health Technology Assessment and Assistive Technology, which not only help the patient or the care provider identify the exact issue with the former, but also identify their specific support needs.
