You have been asked by Jose Lopez, a VHA Administrator, to provide your recommendation regarding the most cost effective care strategy to implement in his new clinic in the rural Southwest.

ent VA population and future projections for growth in his region, along with the continuum of care models and the latest statistics on the number of users by the type of program. Veterans ages 65+ cu
ently make up 60% of his enrolled clinic patients. His review determines that veterans who use institutional care (avg $ per user = $61k) cost on average three times that of veterans that use home or community based services (avg $ per user= $21k).

Jose understands that the aging population will likely increase the demand for future services and with budget funding not anticipated to grow at the same rate he needs to determine which of the VHA’s four strategies to overcome public health challenges associated with the aging population might work best for his clinic.   The VA strategies he has asked you to review include: targeting, training, teams and technology.

Introduction
In future we are planning for non-institutional that is home based primary care for the long term care which is developed in the Department of Veterans Affairs in which they will be on ongoing care to home bound person. Some reports shows that, the rural people of American Indians have restricted facilities are designed for feeble older adult. There are some centre of Veterans Affairs Medical Center have started attempt to enlarge access to Home based primary care with the help of local tribes and Indian Health Service facilities. The data proved that more than 10 million people of old adult age need long term care to be in house or I in community based facility which compared with around two million people in the organization or institution where nursing facilities or chronic care hospitals are available.
The need of home care or in person assistance in day to day life activities increase with the age. 
The statistics shows that in the year of 1990-1991 around 50 percent of the civilian’s non institutional populations were 85 years and the people which was the older, aged between 65-69 years.  Statistically, this type of people, where 75 percent were 65 years older or more, female about 65 percent or white was 68 percents. 
Referring the Veterans Affairs health care services which admit the veteran who are seek and need care. Not even this, in some cases the contracted care acquired from the people who need care for long term. 
The Institutional facilities may comprise inpatient emergency care and nursing home care. The most of VA Community living Centers, state veterans home, community nursing home etc.
The Non institutional can be outpatients and ambulatory care settings, also which happens in the house and it may be in another group.
There may be some non institutional services are :
Home based primary care, community residential care, geriatric evaluation, palliative care, adult day health care, homemaker or home health aide care, respite care, home skilled care, home hospice and veteran directed home and community based services and medical foster homes.
The socio economic data shows that, According to data the American Community Survey, just we able to find that there are less number of unemployed veteran’s person, they are very less in number which is living in below poverty line, and the most important things that they are educated at least they are Graduate.   They have medical insurance.
5) Summarize how the population has aged and costs have increased for institutional – hospital and skilled nursing home services
Summary: 
The long term care is needed when the person is suffering from chronic disabilities that damage the capability to do any kind of activities which is necessary in day life.
