Overview
According to WHO, Global Health is an area of study, practice and research focusing on improving the health and achieving health equity for people across the world (World Health Organisation, 2019). Global Health ethics includes the administration of moral values to the issues related to public health (Stapleton, 2014). The six principles of Global Health Research involve Authentic Partnering, Inclusion, Shared Benefits, Commitment to the future, responsiveness to causes of inequities and humility (Canadian Coalition for Global Health Research, 2015). All these principles promote equity in global health and encourage greater governance for supporting the health research.
The Global Health ethics include the topics like the discussions in terms of choices of treatment, and the care options faced by the healthcare professionals and people, the relationship between care providers and patients, the resources, structures and systems used to improve the health, ensuring ethical conduct in research, determining whether to allocate funds to the promotion of health or treatment of disease etc (Nova Scotia Health Ethics Network, 2019).
Analysis of Ethical Dilemmas
In case study 1 the participants of the study were sued for the trial for one year and left after that without providing adequate complementary healthcare, access to free medicines or compensations. The research was not conducted ethically and did not protect the participants who were vulnerable and least privileged citizens. They were left with undetectable viral loads after the trial. The patients were not appropriately notified that they will be taking part in study only for one year and after that they will be left without any drugs after completion of study. The consequential acceleration of AIDS for the patients was left unaddressed. The intervention was not at all useful for the participants during the course of trial.
In developing countries, the limited health care services, poverty, cultural and linguistic differences, illiteracy and reduced level of understanding about the scientific research increase the incidence of such exploitations (Emanuel, Wendler, Killen & Grady, 2014). In developing countries, the independent oversight processes and regulatory framework are less financially supported, less well established and are less effective. The ethical guidelines for research need to reduce the risk of exploitation in such cases.
The Tuskegee Syphilis research also illustrates unethical observational research that harmed and exploited the most vulnerable groups of population. The ongoing research did not provide effective medicine to the participants with Syphilis even after it became available.  The study goals were not informed to the participants and nor they were informed about the alternate treatment options available outside the research. They were told that they would be provided free healthcare from US government. The participants were told that the study would go on for just 6 months, however it lasted for 40 years.
