This includes analysing medical and situational factors that contribute to a complex health care scenario.Once this has occurred Registered Nurses and other members of the health care team can initiate complex care planning commencing with analysis of the risk of further harm/hospitalisation, identification of the individual’s goals and any barriers to care including conflicting treatment approaches for client with multiple medical conditions.Task:The newly formed Youbeaut clinic is currently in the process of applying for the federal governments Healthcare Home Program (https://www1.health.gov.au/internet/main/publishing.nsf/content/health-care-homes-professional). In order to show the ability of the new practice to undertake this program, the healthcare team has already adopted some of the practices associated with this program namely, the use of a screening tool to identify complex patients who would be considered eligible for the program. The practice has been able to create a RN position to act as care coordinator for these patients. Ben is at the Youbeaut clinic who meets the inclusion criteria. Introduction
Medical complexity is the presence of multiple substantially important chronic health issues in a single individual. As supported by Kuo and Houtrow (2016), it results in affecting multiple organ systems and extreme need for frequent healthcare support with advanced technologies. It may lead to several functional impairments and there are chances of being combined with other mental disorders such as trauma, anxiety, depression and so on. They are in continuous need for essential healthcare services for survival.
They are termed “medically fragile”. There is an utmost need to handle such cases with proper care that the patients actually need to keep them mentally strong enough to fight through all odds. As opined by Mitsi, Kourakos, Poulimenakou, Latsou and Sarris (2018), a proper therapeutic treatment if imparted towards such patients with the help of effective nursing may help to alleviate their chronic pain by opening up and accepting their body with all its medical complexities.
1. Elements Associated with Medical Complexity of Ben
	Ben has been diagnosed with having many diseases together as per his case study and different illness calls for a need of separate treatments altogether. Ben was initially working for the Australia Defence Force (ADF) when he was severely injured in his spine during training leading to LA-L5 fusion in the vertebral column. Since then, he developed a chronic pain in his lower back. He is diagnosed of having Post-traumatic stress disorder (PTSD), which developed along with his back injury.
He had continued for a while with restricted duties, which made him feel more depressed than before and finally left the ADF for his health issues, he had taken employment in regional mine. The job did not go quite well since Ben had to hide his health impact due to continuous sitting at a place and frequent shaking of mining equipment elicited his lower back pain as well as episodes of PTSD. He resorted to taking painkillers in order to sleep during night and work during day.
He was unable to manage the heavy machinery and lost this job suddenly as well. He was unable to find proper employment and resulted in development of depression in him. Ben, on his return back to his farm and reuniting with his family was turning more into an alcoholic. He was receiving proper psychological treatment, which were mostly dedicated towards cognitive behavioural therapy, since many years and was effective for him. He was more affected by depression than any other physical ailment.
Due to this situation, his inability to find himself employed even after serving the defence force for 10 years kept playing in the back of his mind. He had resorted to exercising to combat his depression, as supported by Kandola, Ashdown-Franks, Hendrikse, Sabiston and Stubbs (2019) and PTSD better but his chronic back pain provides him the distress when he goes to gym. Hence, he is not attending gym on a regular basis after staying back with his family, which may trigger his mental imbalance. 
2. Conflicting Management Elements in Nursing Care of Plan of Ben
As Ben is suffering from multiple disorders or his case is one of a medical complexities, he has different treatments and care dealing with his different ailments. He needs to get rid of his chronic back pain, which is causing his inefficiency in getting and doing proper jobs. Simultaneously,
