Case Study One- Sam
The case of Sam is an indeed complicated one as she is suffering from kidney failure in which her longevity also will be for few more years and living upon dialysis until and unless there is any organ donor to her. In addition, there are several ba
iers in getting the best care, which she actually deserves. The root cause of the problem needs to be analysed after its identification, which is Sam being an elderly woman is losing her patience in continuing the treatment, as she needs to travel great distances every week in order to get the dialysis done.
As supported by Haahr, Norlyk, Martinsen and Dreyer (2020), the elderly patients need much more care and the nurses must be a lot sensitive in handling them and here, conflicting situations arise out of the ethical issues related to the patients where there is an utter confusion between the free-will and the good-will of the patients. The patient herself is tired of the ongoing process of her treatment, which is quite natural at her age and even the treatment process, dialysis, being painful but she needs to pursue it in order to live longer (Moranne et al., 2018). As supported by Schadewaldt, McInnes, Hiller and Gardner (2016), this may give rise to any fatal situation concerning the ethical issues and the right decision at the right time needs to be taken concerning ethics in healthcare. Nurses are highly responsible for catering the proper needs for elderly people (National Association of School Nurses, 2016).
At this instance, it is necessary to make the patient understand the importance of the treatment. It is also necessary to tell the patient’s family that as the distance is the main ba
ier in seeking the proper treatment, therefore, they can admit her in the hospital or any nea
y aged care home, so that her treatment process is not hampered. Besides, she will remain hale and hearty. According to Gholamzadeh et al. (2018), the essence of empathy amongst the nursing students as a way of care implementation towards the elderly people who is going through huge amount of pain is the most effective way to care for them.
The nurses would try to have proper empathy for the elderly patients and hold all the feelings of pain and anguish caused in them with utmost care. It also depends on the reaction of the patient whether she will actually accept the proposal of staying away from the family, which mostly people at the elderly age, do not intend to. In such case, the student nurse will learn how the registered nurse with her communication skills, convinces the patient’s family regarding the same as this will somehow benefit the health of the patient effectively and allow her to rest more than before.
This is actually the essence in building a therapeutic communication Fite, Assefa, Demissie and Belachew (2019), which directs into creating a positive health outcome of the patient by the nurse’ effectiveness. The student nurse would begin establishing such a relationship with Sam and her family by winning over her with her positive attitude, later on it can create building her closeness with the patient and lead to her health improvement by making her stick to the treatment and undergoing dialysis.
As supported by Nursing and Midwifery Board of Australia (2020), the second principle of the decision making as per NMBA standards declares that moulding the beliefs of people for their own welfare seems to be a next to impossible task which has to be achieved by the medical team frequently and in Sam’s case, this needs to be strategically done for her own betterment.
