Introduction
National Primary Health Care (NPHC) Strategic Framework is basically a nationally agreed policy that has been developed by Commonwealth, states and territory health department of Australia. The framework was launched and approved by the health ministers of Australia in 2013. This frame takes a broad approach and a comprehensive view of primary health care in Australia.
Further, it focuses upon the role of General Practitioners (GP) in primary health care. The current analysis is for one of the strategies from NPHC framework and based upon that, further discussion will be done. Further, significance of nursing practice in primary health care will be discusses with respect to selected strategy. 
Identification of Current Key Strategies in the National PHC Strategic Framework
In order to analyses one of the current NPHC policies, Strategic Outcome 3 ha been chosen. The chosen strategy states to tackle and take action upon the social determinants of health and individual’s well-being with special emphasis upon the prevention, promotion, screening and early intervention. In order words, this strategy states and focuses upon the fact that community or individual’s wellbeing can be promoted.
This can be done by implying a systematic approach at national, state as well as local levels towards the factors that affects health status and receive better health outcome (Department of Health, 2019). Further, this strategy also states that in order to improve the health condition of individuals within a state or local are it is necessary to focus upon social determinants of health because negligence towards them can be one of the major factor behind poor health conditions. 
Discussion upon Determinants of Health
Major determinants of health included within this strategy are social status, geographical location, health literacy, education, housing, employment and access to health services.
