Case Study 1
Robert Williams, 52-year-old, African- American male was admitted to emergency department (ED) yesterday evening. Robert reported sudden acute chest pain, with stiffness in his chest. Further, he also reported shortness of
eath, anxiety, dizziness and fast heart rate. Along with these presented symptoms, Robert experienced jaw pain and lightheadedness. Robert was playing with his granddaughter and was trying to catch her before he fell on the fall and reported prior presented symptoms. Robert presented that he had a family history of high cholesterol and it could be genetic in his family as his mother and father both suffered from high cholesterol. Further, he was diagnosed with high cholesterol at the age of 24 and is on medications since then. Moreover, at the age of 29 Robert was diagnosed with diabetes mellitus type 2, which again could be a genetic disease as evident from his family background.
Robert’s full set of observation include:
Body Temperature – 37.1C
Pulse Rate – 140 beats per minute
Respiration Rate – 22
Blood Pressure – 140/90 mm Hg
Blood Sugar Level – 320 mg/dL
Along with these presented symptoms, Robert’s skin appeared to be pale yellow, his
eath was shortened, faced difficulty in speaking or moving. Likewise, Robert also reported jaw pain, nausea, high levels of sweating. Moreover, ECG reports for Robert reflected a pathological Q wave, which also reflects a previous sign of myocardial infarction or heart attack; however, surprisingly Robert and his family report that this was the very first incident of myocardial infarction reported in Robert.
Multiple Choice Questions with Solutions and Justification
Q1.
What could be the facilitator of myocardial infarction in Robert?
a. Robert’s ethnicity
. Robert’s age
c. Body temperature
d. Prior history of cholesterol and diabetes
Answe
d. Prior history of high cholesterol and diabetes could be considered as the right option for this question as supported by Kim et al. (2017) high cholesterol na
ows down blood capillaries; thus, facilitating heart attack.
On the other hand, ethnicity and body temperature are not related to myocardial infarction; however, increasing age could be presented as facilitator yet, is not specific (Beheshti et al. 2018).
Q2.
Robert’s condition can be recovered and managed through
a. Medication and proper diet
. Only medication and no focus on diet
c. Use of analgesic
d. Robert can recover with time
Answe
a. Robert required a combination of interventions including proper medication such as aspirin or blood thinners along with diet low in fats and bad cholesterol (Molenberg et al. 2017). Likewise, only focusing upon medications and consumption of poor diet will not help. Use of analgesics will further facilitate relaxation of blood vessels and increased risk of heart attack.
