Aged Care Services/Packages
Home Caring
Home Caring was founded in 2015 with the aim of delivering quality in-home care that is centred on the patient and their requirements. They are committed to delivering all clients' professional and competent treatment and service. They provide support and care to the elderly to assist them to feel more independent and confident while also increasing their quality of life outside and in the home. They have specialists available to support people in managing their diabetes in residential care (Ancion et al. 2019).
Home Caring service will provide them with the best possible care. Their carers will be there for them physically as well as emotionally, from communication and support to food preparation that meets their appropriate dietary demands (Jordan et al. 2019). Dietitians and physicians will tailor a meal plan for them, whether it's minimal salt, reduced cholesterol, minimal fat, vegan, or vegetarian, and their caregivers will make sure they stick to that plan. Not only that, but they will be capable of assisting the family to assist them with managing their diabetes care, including medication administration and physical activity promotion, to ensure they are in the best condition possible (Do
ică et al. 2019).
Concerning the case of Flo, she is already suffering from diabetes and has been limping and seems to be underweight. This may be due to her improper diet and self-care. Therefore, Home Caring can support Flo in managing her diabetes. They will also help Flo with her foot care as well as assist in having a proper diet (Mulchandani & Kakkar 2018). This may assist her in managing her body weight. Her limping may also be addressed with their services as they will help her with physical activity promotion. They make sure that all professional caregivers are educated to identify any concerns that may cause long-term harm, such as ingrown nails and wounds. Professional caregivers can evaluate the feet regularly to ensure no harm has occu
ed (Reynolds et al. 2022).
Flo will not have difficulty accessing this service as she is already living in a town where she can access the Home Caring services by contacting them on call and sending mail to a given mail-id (Skibitskiy et al. 2020).
Anglicare
For nearly 90 years, Anglicare has provided community care and services to the community. Anglicare has its roots in community care around 150 years ago. Their purpose is to help needy people, enhance people's lives, and spread the love of the Lord. It is a home care service that promotes independence by providing tailored in-home care facilities. Their professional group of home care experts will assist individuals and they will get comprehensive training and continuous assistance to provide a high quality of service. They have been providing aged care in the elderly's residences for over 75 years (Jordan et al. 2019).
As Flo has a medical history of angina, hypertension, hypercholesterolemia, type 2 diabetes, osteoarthritis, asthma, and a history of falls, Anglicare can provide a specialised falls prevention program. They will also provide an occupational therapist to help her with foot wound care. As Flo is limping, a physiotherapist would assist her to address this issue (Ancion et al. 2019). As Flo and her husband appear to be underweight, a nutritionist may help her to maintain their healthy diet and manage their weight. Podiatrist services are also provided by Anglicare, thus foot care for Flo can also be managed. Their residential aged care services and nursing homes have been carefully developed to provide person-centered care in a residence-like setting while simultaneously promoting individual autonomy. They aim to help individuals continue to live a purposeful and satisfying life by letting them know their background, ho
ies, and preferences. Their Community Pantries and Mobile Community Pantries at selected Anglicare Op Shops may also offer minimal-cost food aid (Ba
y, Greenhalgh & Fahy 2018).
Flo can easily access this service. Their allied health services are provided to all individuals and may be accessed from a variety of villages. Some communities also have consultation rooms available for medical visits. Flo may find out what services are available in her local town by contacting them, and individual visits can be scheduled through her healthcare professional. Their home care patients can also communicate with their team of health care professionals and specialists, assuring a continuum of care as their requirements and situation change (Jordan et al. 2019).
