DESCRIPTION OF TASK
The overall aim of the assignment is for students to gain a greater understanding of how to summarise assessment information in a meaningful way and gain practice in documentation skills. It also provides an opportunity for students to propose appropriate interventions for an individual and discuss their approach with a fellow student.
You will be provided with a case study and will be required to develop a formulation matrix, write a case formulation, propose appropriate interventions, develop a person-centred care plan, and compare your plan for working with the person with a student from a different discipline background.
MARKING CRITERIA
This assignment will be assessed against the following marking criteria:
1. Ability to develop a formulation matrix for an individual using the 4 P model and the biopsychosocial model (8%).
2. Ability to write a case formulation (8%).
3. Ability to present a logical and persuasive argument for proposed interventions based on the formulation and academic literature (10%).
4. Ability to document a person-centred care plan for an individual (8%).
5. Ability to compare the similarities and differences in interventions proposed by yourself with a practitioner from another discipline (8%).
6. Ability to write clearly and concisely and use referencing and formatting consistent with APA 7 style guidelines (8%).
A separate marking criteria sheet has been produced for this assessment item so please make sure to read it so you are sure you are on track with your writing.
TIPS FOR COMPLETION
· The client report should be written from the perspective of a mental health practitioner working in a mental health service. An individual has been referred to you and you have been asked to complete a holistic assessment and develop a formulation and person-centred care plan with appropriate interventions for the person.
· When summarising a person's socio-demographic information you can discuss their age, gender, marital status, number of children, financial status, transportation, who they live with, employment etc.
· When summarising the person’s presenting issues make sure to state what the person sees as the presenting issues. Thoughts, feelings, and perceptions related to any symptoms should be noted together with any screening or diagnostic assessments that have been conducted.
· For assessment item 3 you are not required to provide a provisional diagnosis for the person in the case study.
· Topic 8 has information on developing formulations and writing care plans. Make sure to read through Topic 8’s content, work through the activities and check your answers against the example formulation matrix and care plan provided for George.
· In Topic 8’s Preparation for Assignment 3 section you will find 4 documents that you should find useful for your write up.
· Topic 8 also includes a recovery-oriented language guide. Make sure to refer to this guide when writing up your assessment. You will lose marks if you don’t use recovery-oriented language.
· When presenting your proposed interventions make sure to base these on your assessment findings and the person’s goals. Literature should be used for identifying priorities and interventions for the person.
· There is no set number of interventions to include. It depends on what you plan to focus on with the person and what would be manageable for them.
· As noted in Topic 8, when considering the types of interventions to include in your person-centred care plan, you can explore a range of options including:
· professionally delivered clinical interventions such as medications or therapy, including evidence based practices where applicable.
· self-help and peer support.
· exercise and nutrition guidance.
· rehabilitation and skill-building to enable the person to live in the least restrictive environment.
· daily maintenance activities and plans for managing symptoms and behaviours before they get worse.
· spiritual practices and affiliations.
· homeopathic and naturopathic remedies.
· cultural healing practices/involvement of Indigenous healers.
· increased involvement in community activities or connections with natural supporters.
· rehabilitation opportunities such as supported housing, supported education, supported employed and supported community living.
· practical assistance in community contexts to address basic human needs for housing, food, work and connection with the community.
· Please make sure to use literature to support your ideas for marking criteria 3 and 5. You will need to draw on academic journal articles and books (not websites). You can access journal articles and books either via the Griffith University library catalogue or googlescholar. To obtain a higher mark you need to be critically analysing the literature.
INTERVIEWS
· Students are asked to interview a student from a different professional background to discuss their approach to working with the person. This is similar to when we talk with other members of an interdisciplinary team to discuss issues and share ideas about how we can work with someone.
· If you aren’t working in the mental health area, please make sure to interview someone who is.
· To assist you to find a student to interview, we will be setting up a Wiki where you can provide your contact details and information about which case study you are working on. Once it is set up, click on the “Wiki – finding fellow students to interview for Assessment Item 3” heading in Assessments 3’s folder.
· You can do your interview via phone or Microsoft teams. For those who have not used Microsoft teams before there is information listed on the course site about how to use it.
· Your discussion around your interview may include the similarities and differences in your approaches, frameworks drawn upon, priorities for working with the person, as well as proposed interventions and techniques for working with the person. Literature needs to be used to support your discussion.
· Some questions you might want to consider asking include:
· How does your discipline typically work with people?
· What priorities did you have for working with the person?
· What interventions did you propose for your care plan?
· What informed your choice of interventions (eg. theories/frameworks/literature)?
· When discussing the approach taken by the other student, the similarities and differences to yours, make sure to use the literature to support your points. I have listed an example below for how you could write your discussion:
“X was from a nursing background and talked about practicing from a XXXX perspective when working with people (reference). They thought the key interventions to focus on with XXXX were XXXX and XXXX as …….. The use of XXX has been found to be a highly effective intervention for people presenting with XXX (reference). They also thought the use of XXX might be a good idea as the person had expressed the goal of XXX. According to Brown XXXXXXXXXXthis would be a suitable intervention as ….. After talking with the other student, what is common to our approaches to this case study is the use of XXXX....... According to Smith (2014), this XXXXX approach is associated with XXXXXXXXXXWith respect to the differences between our approaches, X took a more XXX approach while I utilised XXXXXXXXXXwhich may result in ……”
· Please reference your interview as a personal communication (see the APA 7 referencing guide for how to do this).
· Students are encouraged to respond to student requests for interviews as promptly as possible – each student needs to make contact with others to complete this assessment piece, so it is important that we all work together to meet this requirement.
· If you cannot find another student to interview you can also approach people working in the mental health area but make sure they are from a different discipline to you.
DOCUMENT FORMAT
· The client report is not to be written in essay format but instead is to be written in 5 sections with a heading for each section.
· Section 1 - Formulation matrix
· Section 2 - Case formulation
· Section 3 - Argument for proposed interventions
· Section 4 - Person centred care plan
· Section 5 - Interview with practitioner from another discipline
· Formulation template: Use the formulation template listed in the assessment folder and make sure to assign factors correctly to each of the sections. If you think there are no factors for a particular area of the formulation matrix then just leave it blank.
· Care plan template: Use the care plan template listed in the assessment folder. Make sure to align your goals, objectives and interventions for the person.
· Make sure to use double line spacing, indent the first line of your paragraphs and have no line spaces between paragraphs (correct APA 7 format). Ideally use size 11 or 12 Arial or Times New Roman font.
WORD COUNT
· Make sure not to go over the word count as the markers will stop reading once they read the 2,500 word limit.
· It is a tight word limit for what you need to cover. If you are finding you are over the word limit then please watch the Tips for Writing to Word Counts video which is listed on the assessment section of the course site as it will help you being more concise in your academic writing.
REFERENCING
· There is no recommended number of references for this assignment. It is more about making sure to reference to the literature in your work and to try and use more recent literature (ie. in the past 5 to 10 years). You don't need references in your matrix, formulation or care plan. Referencing is needed though in your discussion of proposed interventions and in your comparison with another discipline about how they would work with the person in the chosen case study.
SUBMITTING YOUR ASSIGNMENT
· You will need to submit your assignment via the course site using Turnitin. There is a draft submission point in the Assessment 3 folder that you can use to check for plagiarism (if you are concerned about this). In busy study periods it can take a while for the draft checks to be processed so please make sure to give yourself at least a day for this to occur.
· Please submit your assessment in Word format to allow for the insertion of comments and feedback. Make sure not to submit a pdf document.
· Make sure to complete the electronic cover sheet (do not add a cover sheet to your essay only a title page that includes your name, student number and word count).
